Detection And Correlation Analysis Of Microvessel Density And Nerve Fiber Layer Thickness In Optic Disc Before And After Anti-Vascular Endothelial Growth Factor Treatment In Patients With Retinal Vein Occlusion

Objective:To observe the changes of radial peripapillary capillary vessel density(RPCVD)and nerve fiber layer thickness(RNFL)in optic disc region of monocular patients with branch retinal vein occlusion(BRVO)complicated with macular edema(ME),and to analyze the relationship between RPCVD and RNFL after treatment with anti-vascular endothelial growth factor(VEGF).Methods: 47 patients who were diagnosed as monocular BRVO-ME and received anti-VEGF treatment in the Department of Ophthalmology of the first affiliated Hospital of Bengbu Medical College from August 2019 to December 2020 were selected.Optical coherence tomography angiography(OCTA)was used to evaluate RPCVD and RNFL.The RPCVD and RNFL,of the BRVO group,the contralateral healthy eye group and the normal control group before treatment were compared and analyzed,and the correlation between RPCVD and RNFL in the contralateral healthy eye group was analyzed.The changes of RPCVD and RNFL in BRVO group before and after treatment were compared and analyzed.The changes of RPCVD and RNFL in BRVO group after treatment and those without re-injection were compared and analyzed,and the correlation between RPCVD and RNFL in BRVO group was further analyzed.Result:1.before treatment,the whole image RPCVD(45.17 ±2.33)%,inside disc RPCVD(45.38 ±2.09)% and peripapillary RPCVD(45.85 ±1.68)% in the BRVO group were lower than those in the contralateral healthy eye group [(48.9 ±2.50)%,(47.4 ±3.41)%,(49.3 ±1.50)%] and the control group [(50.13 ±3.38)%,(49.38±3.38)%,(51.12 ±1.82)%](all P < 0.05).The RPCVD in the whole image,inside disc and peripapillary in the contralateral eye group were lower than those in the control group(all P < 0.05)2.The average thickness of RNFL in the BRVO group(102.67 ±1.43um)was higher than that in the contralateral healthy eye group and the control group.(all P< 0.05)The average thickness of RNFL in the contralateral healthy eye group was lower than that in the control group.(P < 0.05).3.In the contralateral healthy eye group,the average thickness of RNFL was positively correlated with the whole image,inside disc and peripapillary RPCVD(r = 0.485,r = 0.442,r = 0.470,P < 0.05).In BRVO group,the whole image RPCVD(46.53 ±1.73)%,inside disc RPCVD(46.10 ±1.89)%,and peripapillary RPCVD(46.96 ±1.53)% increased after treatment(all P < 0.05).The average thickness of RNFL in BRVO group after treatment was(99.44 ±1.47)μ m,which was lower than that before treatment(P< 0.05),but not lower than that in control group(P < 0.01).4.The RPCVD in the whole image,inside disc and peripapillary in the BRVO group without re-treatment was(46.32 ±1.68)%,(46.20 ±1.90)% and(46.68±1.57)% respectively,which was not significantly different from that in the BRVO group after treatment(all P > 0.05).The average RNFL thickness in the non-treatment group was(96.73 ±1.62)μ m,which was lower than that in the BRVO group after treatment and that in the control group(P < 0.05).5.In the non-treatment group,the whole image RPCVD was weakly positively correlated with the average RNFL thickness(r = 0.408,P < 0.05),and the RPCVD inside disc and peripapillary was not significantly correlated with the average RNFL thickness.(r = 0.42,P > 0.05;r = 0.289,P > 0.05)Conclusion:(1)OCTA can quantitatively analyze the RPCVD and RNFL thickness information of BRVO.(2)The RPCVD and thickness of RNFL in contralateral healthy eyes decreased,and there was a significant correlation.(3)The RPCVD of the eyes with BRVO decreased more obviously,and the RPCVD of the eyes increased after anti-VEGF treatment.The RNFL edema thickened before treatment,and the RNFL thickness decreased after anti-VEGF treatment,which is considered to be related to the regression of edema.(4)6 months after the onset of BRVO,the thickness of RNFL decreased,which was weakly correlated with RPCVD.
