Study On The Clinical Experience Of Kang Liangshi In The Treatment Of Tympanites(Hepatitis B Liver Cirrhosis Ascites) Based On The Ancient And Modern Medical Cloud Platform System

Objective:This study collects and summarizes the medical records of Professor Kang Liangshi’s treatment of tympanites,and uses data mining methods to analyze and summarize Professor Kang’s medicat-ion rules and experience in the treatment of tympanites.Methods:This study mainly selected the outpatient medical records of patients with tympanites who were treated in the outpatient clinic of Professor Kang during1994-2006.According to the inclusion and exclusion criteria,100 cases were selected for inclusion,with a total of 373 visits.After prescription calibration,data entry,and standardization processing,frequency statistics methods are used to make statistics on basic conditions,four-diagnosis data,frequency of use of traditional Chinese medicines and their attributes and efficacy classification;use association analysis,chi-square test,and complex network analysis to mine and analyze drug combinations,drug pairs,and core prescriptions for the treatment of tympanites.Results:(1)Among the cases included in this topic,the top eight clinical manifestations are ascites,abdominal distension,jaundice,swollen feet,oliguria,diarrhea,facial edema,and fatigue.(2)The top ten Chinese medicines with high frequency of use are:Polyporus,Oriental Waterplantain Rhizome,Chinese Lobelia Herb,Turmeric Root Tuber,Poria skin,Corn Stigma,Orthosiphon,Cape Jasmine Fruit root,Officinal Magnolia Bark,and powdered Notoginseng.(3)In the analysis of Chinese medicine properties,the four qi is mostly flat tastes,except for the flat tastes,from the perspective of the four natures-cold and heat,cold drugs are more than warm’s.For the five flavors,sweet drugs are the first,followed by bitter,mild and pungent flavors.The return meridian mainly includes the lung meridian,stomach meridian,spleen meridian,liver meridian,kidney meridian,etc.(4)In terms of the classification of the efficacy of traditional Chinese medicines,there are nine sub-categories with a frequency of ≧150,but they are mainly concentrated in five categories such as water-disinhibiting medicinal,qi-regulating medicinal,heat-clearing medicinal,blood-activating medicinal,and tonifying medicinal.(5)Using the correlation analysis method,set the support degree ≧0.20,the confidence degree≧0.79,eliminate duplicate combinations,and obtain fifty six drug association combinations.The analysis found that Polyporus,Oriental Waterplantain Rhizome,Turmeric Root Tuber,Poria skin,Chinese Lobelia Herb,Corn Stigma,Orthosiphon,Cape Jasmine Fruit root,and Officinal Magnolia Bark have strong correlation.There are also Amber powder-Notoginseng powder and American ginseng powder-Notoginseng powder,with high confidence,indicating that the drugs in the combination are highly correlated.(6)Use the chi-square test to analyze comorbidities and traditional Chinese medicine,and get the combination of treatment comorbidities.Strongly related are diarrhea-Gallus Gallus Domesticus,Chicken’s egg Membrane,hemorrhage-Beauty-Berry Leaf,Agrimony,pleural fluid-Pepperweed Seed,White Mulberry Root-Bark,skin itching-Puncturevine Caltrop Fruit,Cicada Slough.(7)Use complex network analysis to get the core prescription:Chinese Lobelia Herb,Polyporus,Oriental Waterplantain Rhizome,Orthosiphon,Cape Jasmine Fruit root,Corn Stigma,Poria skin,Turmeric Root Tuber,Virgate Wormwood Herb,Officinal Magnolia Bark,Coix Seed,and powdered Notoginseng.Conclusion:Through data mining and analysis,the following conclusions are drawn.(1)it is concluded that Professor Kang believes that the cause of bloating is due to stagnation of qi,dampness obstruction,heat accumulation,blood stasis,and healthy energy deficiency.It is proposed that the airway is blocked and the tunnel is blocked,which is used to explain the two trends of the evolution of the tympanites pathogenesis.(2)Professor Kang is based on the core prescription,adding or subtracting medication according to the heaviness of Qi and blood.If the airway is obstructed,the Qi should be regulated and supplemented at the same time.If the tunnel is impassable,strengthen blood circulation and remove blood stasis.If healthy Qi deficiency is not obvious,use Amber powder-Notoginseng powder to cure it;if healthy Qi deficiency is obvious,use American Ginseng powder-Panax Notoginseng powder to cure it.If there are concurrent symptoms,use a special medicine pair or group to treat the symptoms.(3)Professor Kang uses mild medicine to treat tympanites,using more dredging methods and using less the method of drastically purgating.Use more mild and sweet drugs,rarely use drugs for severe cold and fever,and rarely use large doses of drugs.Specially like to use local herbs in southern Fujian,which has local characteristics.
