The Clinical Observation Of Abdominal Acupuncture In Female Late-onset Acne With Deficiency Of Liver And Kidney Yin

ObjectiveAccording to RCT,this research is aim to observe the effectiveness of abdominal acupuncture,compare its advantage,safety and psychological intervention effect with isotretinoin,and provide clinical dates,evidence for the treatment of female late-onset acne with deficiency of liver and kidney yin.Methods65 patients,who were diagnosed as female late-onset acne with deficiency of liver and kidney yin,were recruited from the first affiliated hospital of Guangzhou University of TCM and Guangzhou Hospital of TCM,and divided into the acupuncture group and drug group randomly.32 patients underwent acupuncture treatment twice a week for eight weeks,which selected Zhongwan,Xiawan,Qihai,Guanyuan,Qixue,Huaroumen,Wailing,Daheng(right single one),Shangfengshi(right single one),Yindu as acupoints.33 patients received isotretinoin treatment with 20mg bid dose in the initial four weeks and 10mg bid dose in the last four weeks.By the time,both of them received Chinese medicine mask and basic treatment.During the period of treatment,all the patients had to complete the evaluation of GAGS and DASS-21,and marked the adverse events if necessary.ResultsThe study totally collected 30 effective cases each group,with 2 shedding cases each group and 1 excluding case in drug group.Baseline comparison:Before the treatment,there were no differences in age,disease course,total lesion score,local lesion score,acen grade,DASS-21 score and grade(P<0.05)between thw two groups,which showed their comparability.Lesion comparison:Within each group,there were significant differences in total lesion score,acne grade,local lesion score before and after treatment(P<0.05).Between the end and follow-up visit of the treatment,there were significant differences in total lesion score(P<0.05),and no difference in acne grade.Moreover,there were no significant differences in the left cheek,chest and upper back lesion score in the acupuncture group,and the nose lesion score both two groups(P>0.05).There were significant differences in the right cheek lesion score in the acupuncture group(P<0.05,with negative mean),and the rest lesion score(P<0.05,with positive mean).Between the groups,there were no significant differences in total lesion score variance,acne grade,facial lesion score variance(P>0.05),and significant difference in the chest and upper back lesion score variance(P<0.05)before and after treatment.At the end of treatment,the total effective rate of the acupuncture group and drug group was 83.8%and 80.0%respectively,which showed no significant difference(P>0.05).In follow-up visit,the long-term effective rate of the acupuncture group and drug group was 76.7%and 46.7%respectively,which showed significant difference(P<0.05),and there were significant differences in total lesion score variance,and each cheek,chest and upper back lesion score variance(P<0.05),no significant differences in acne grade and forehead,nose,chin lesion score variance(P>0.05).Psychology comparison:Within each group,before and after treatment,there were significant differences in depression,anxiety,stress score and anxiety grade(P<0.05),no significant differences in depression and stress grade(P>0.05).Between the groups,after treatment,there were no significant differences in depression,stress score variance and both psychological grade(P>0.05),and significant difference in anxiety score variance(P<0.05).ConclusionBoth two treatments do improve the acne lesion,alleviate depression,anxiety,stress in patients with deficiency of liver and kidney yin type of female late-onset acne.Isotretinoin shows more significant improvement in the chest and upper back lesion,comparatively,abdominal acupuncture is better in long-term effevtiveness,especially for each cheek and chest and upper back lesion.Meanwhile,abdominal acupuncture shows more visible alleviation of anxiety.
