Objectives:To explore the risk factors leading to the expression of human papillomavirus(HPV)E6/E7 mRNA by analyzing the indicators of vaginal micro-ecology of women in Qujing area,and to timely find and regulate the status of vaginal microecology imbalance,so as to provide theoretical evidence for the occurrence and development of clinical prevention and treatment of cervical disease in the future.Methods:From December 2019 to August 2020,276 women of reproductive age who came to the department of gynaecology of the First People’s Hospital of Qujing were randomly selected between 21 and 45 years old to voluntarily undergo HPV E6/E7 mRNA detection and vaginal microecological detection.93 patients with positive HPV E6/E7 mRNA were classified into the experimental group.In addition,183 cases with negative HPV E6/E7 mRNA detection were included in the control group.Vaginal secretions were collected from the two groups respectively,and vaginal microecological indicators were detected,including:vaginal microbiotic density,vaginal microbiotic diversity,bacterial vaginosis,vulvovaginal candidiasis,trichomonal vaginitis,aerobic vaginitis,lactobacillus,inflammatory reaction,Vaginal cleanliness,hydrogen peroxide,sialidase,leukocyte esterase,β-glucuronidase,coagulase.Statistical data analysis using SPSS 24.0 statistical software,measurement data with mean age ± standard deviation(x±s)indicated that the use of two independent sample data t-test analysis.Vaginal micro-ecological evaluation indicators are enumeration data,their usage rate(%)is indicated,using the method isχ2 test.Multi-factor correlation analysis using logistic regression for statistical analysis,testing the level α=0.05,P<0.05 to determine the difference is statistically significant.Results:1.Compared with HPV E6/E7 mRNA expression-positive group and HPV E6/E7 mRNA expression-negative group,the age distribution difference was not statistically significant(p>0.05).2.Compared with HPV E6/E7 mRNA expression-positive group and HPV E6/E7 mRNA expression-negative group,vaginal microbiotic density abnormality,vaginal microbiotic diversity abnormality,the number of leukocyte,vaginal cleanliness abnormality rate,VVC detection positive rate,TV detection positive rate,AV detection positive rate,GUS-positive rate,GA-positive rate were larger than the control group,but the difference was not statistically important(p>0.05).3.Compared with HPV E6/E7 mRNA expression-positive group and HPV E6/E7 mRNA expression-negative group,lactobacillus decreased,BV-positive rate,LE-positive rate increased,H2O2-positive rates(H2O2 deficiency)and SNa-positive rates increased,and the difference was statistically significant(p<0.05).4.Multi-factor logistic regression analysis was carried out on variables with statistical differences(BV,lactobacillus,H2O2,SNa,LE)in single-factor analysis,with HPV E6E7 mRNA expression positive as the cause variable.Statistically significant(p<0.05)indicators are BV,lactobacillus,LE,wherein BV and LE are independent risk factors for HPV E6E7 mRNA expression(OR>1),THE OR values are 3.074(1.197,7.896),2.178(1.161,4.084).Lactobacillus is the independent protective factor for HPV E6E7 mRNA expression(OR<1),and the OR value is 0.325(0.179,0.590).Conclusions:1.Vaginal micro-ecological disorders,in particular lactobacillus reduction,hydrogen peroxide reduction,SNa-positive,LE-positive and BV-positive rates were associated with HPV E6/E7 mRNA expression.2.Bacterial vaginosis and LE-positive are independent risk factors for HPV E6/E7 mRNA expression,and they are positively related to HPV E6/E7 mRNA expression.3.Lactobacillus is an independent protective factor for HPV E6/E7 mRNA expression,which is negatively related to HPV E6/E7 mRNA expression,and the fewer lactobacillus in vaginal micro-ecology,the easier it is for HPV E6/E7 mRNA to be positive.4.Regulating the micro-ecological imbalance of the vagina may reduce the infection rate of HR-HPV,which is of some significance for the prevention and treatment of cervical lesions.
