Analysis Of TCM Syndrome Characteristics And Related Factors Of Angina Pectoris After PCI In Elderly Patients With Coronary Heart Disease

Objective The purpose of this study was to retrospectively analyze the medical records of elderly patients with angina pectoris after PCI,and to explore the correlation between TCM syndrome characteristics and sex,age,smoking history,drinking history,homocysteine(homocysteine,Hcy),C-reactive protein(C-Reactive Protein,CRP)and low density lipoprotein cholesterol(low density lipoprotein cholesterol,LDL-C).The purpose of this study is to provide some reference basis and ideas for syndrome differentiation and treatment of angina pectoris in elderly patients with coronary heart disease after PCI.Methods Using the method of retrospective analysis,130 inpatients who met the inclusion criteria were selected from the Department of Geriatrics and Cardiology of the affiliated Hospital of Shandong University of traditional Chinese Medicine.The information of sex,age,smoking history,drinking history,laboratory index,main symptoms and signs of the subjects were statistically analyzed.At the same time,TCM syndrome differentiation was carried out by two or more attending or chief physicians of traditional Chinese medicine,and the statistical data were statistically processed by SPSS26.0 software,and the results were analyzed systematically.Result 1.Among the 130 cases included in this study,the main TCM symptoms and signs were chest pain(76.9%),chest tightness(70.8%),palpitation(54.6%),shortness of breath(41.5%),and lack of food and tolerance(24.6%);The frequency of tongue pulse was more than 20% : purple dark tongue,petechia,thin white coating,heavy pulse,fine pulse and astringent pulse;2.The TCM syndromes of patients were the most common syndrome of Qi deficiency and blood stasis,accounting for 40% of the total number of patients;3.Among the 130 cases,47% were female and 53% were male,and there was no statistically significant gender difference among different syndromes(P>0.05).4.Among the patients included in the study,the number of patients aged from 60 to 69 years old was the most,and there was significant difference in the average age between heart and kidney yang deficiency syndrome and cold coagulation heart pulse syndrome,phlegm turbid internal obstruction syndrome,qi deficiency and blood stasis syndrome,qi stagnation and blood stasis syndrome(P<0.05).5.There was no statistical significance in smoking history and drinking history among all syndromes(P>0.05);6.Logistic regression analysis showed that Hcy,CRP and LDL-C were closely related to the classification of TCM syndromes;7.There were statistically significant differences in the mean Hcy values among different TCM syndromes(P<0.05).There were significant differences between qi deficiency and blood stasis and cold coagulation heart arteries,phlegm and turbidity internal obstruction,qi stagnation and blood stasis,and qi and Yin deficiency,as well as between the cold coagulation heart arteries group and the heart-kidney Yang deficiency group;8.The mean CRP difference among all syndromes was statistically significant(P<0.05),and there were significant differences between Qi-deficiency and blood-stasis group and Qi-stagnation and blood-stasis group and heart-kidney Yang deficiency group;9.There were statistically significant differences in the mean LDL-C among TCM syndromes(P<0.05).The LDL-C level in the cold-coaguling-heart-vein group was significantly lower than the internal obstruction of phlegm and turbidity and the heart-kidney-yang deficiency group,and the level of LDL-C in the heart-kidney-yang deficiency group was significantly higher than that in the qi-deficiency and blood-stasis,cold-coaguling-heart-vein and qi-yin deficiency groups.Conclusion The pathogenesis of angina pectoris in elderly patients with angina pectoris after PCI is still mainly due to deficiency and excess,which is related to dysfunction of viscera,and is mainly characterized by turbid phlegm,blood stasis and qi stagnation.The syndrome of qi deficiency and blood stasis is the most common syndrome in traditional Chinese medicine,and the distribution of each syndrome is different,which has no significant correlation with sex,smoking history and drinking history.The average age of patients with heart-kidney yang deficiency syndrome is the largest,which may be related to the increase of patients’ age and the decline of kidney qi.Hcy,CRP and LDL-C are closely related to the classification of TCM syndromes.There are significant differences in the levels of Hcy,CRP and LDL-C among different syndromes,which can be used as a reference for the objective index of TCM syndrome differentiation and treatment.
