The Distribution Of Tcm Syndrome Elements And Their Correlation With Risk Factors In 262 Patients With Angina Pectoris Of Coronary Heart Disease

Objective: To summarize the distribution regularity of TCM syndrome elements of angina pectoris of coronary heart disease,and to provide reference for clinical prevention and treatment.To study the relationship between TCM syndrome elements and risk factors,so as to provide objective basis for TCM syndrome differentiation of angina pectoris.Methods: From September 1,2020 to November 1,2022,262 inpatients with angina pectoris were selected from the Department of Cardiovascular Disease,the first affiliated hospital of Heilongjiang University of Traditional Chinese medicine.By using Excel table to collect the information of clinical syndrome,to make the integral scale of syndrome-syndrome elements,and according to the threshold value of syndrome elements to diagnose the location and nature of disease,to explore the distribution rule of TCM syndrome elements in patients with coronary heart disease and angina pectoris.A binary Logistic regression model was established to study the relationship between TCM syndrome factors and risk factors.Results:1.In this study,the distribution of TCM syndrome elements in patients with angina pectoris is as follows:(1)There were 8 syndrome elements involved in this disease,which were heart > liver > spleen > kidney > lung > Meridian > stomach > chest diaphragm in order of frequency.The syndrome elements of disease location involve three kinds of combined forms,and the order is: single disease location > double disease location > three disease location.(2)There were 13 syndrome elements involved in this disease,which were Qi Deficiency > blood stasis > Phlegm > blood deficiency > Yang deficiency > yin deficiency > dampness > Qi stagnation > Yang hyperactivity > water stagnation > Heat > cold = moving wind in order of frequency.The deficiency syndrome is mainly qi deficiency,and the solid syndrome is mainly blood stasis.The elements of disease-property syndrome involve 7 kinds of combination forms,which are in order of percentage: double disease > single disease > three disease > four disease > five disease > six disease > seven disease.The frequency of deficiency-excess syndrome is the most,the simple excess syndrome is the second,and the simple deficiency syndrome is the least.2.In this study,the correlation between TCM syndrome elements and risk factors in patients with angina pectoris was as follows:(1)Lipoprotein(a)and diabetes mellitus are the factors that affect the heart of the disease location syndrome.The Heart is positively correlated with lipoprotein(a)and negatively correlated with diabetes mellitus.(2)The smoking history was the influencing factor of the disease location syndrome,and the liver was positively correlated with the smoking history.(3)D-dimer is the influential factor of blood-deficiency syndrome,and blood-deficiency is positively correlated with D-dimer.(4)Hypertension is the influential factor of yin-deficiency syndrome,which is positively correlated with hypertension.(5)Activated partial thromboplastin time is the influencing factor of dampness of pathogenic syndrome elements,and dampness is negatively correlated with activated partial thromboplastin time.(6)Hypertension and Low-density lipoprotein syndrome are the influencing factors of Phlegm,which are positively correlated with hypertension and Low-density lipoprotein syndrome.(7)Sex is the influencing factor of Qi stagnation,which is positively correlated with sex(female).Conclusion: 1.The main pathological changes in patients with angina pectoris of coronary heart disease are heart and liver,the combination of syndrome elements of disease location is mainly composed of single syndrome element heart,liver and multi-syndrome element heart + liver.2.The pathogenesis of angina pectoris of coronary heart disease is mostly characterized by deficiency in origin and excess in superficiality in superficiality,the deficiency of Qi is the main,and the stagnation of blood is the main.The combination of disease-related syndrome elements is mainly composed of double-syndrome elements,and the distribution of deficiency and excess of disease-related syndrome elements is mainly composed of deficiency and excess syndrome.3.There were correlations between TCM syndrome factors and risk factors in patients with angina pectoris,and there were positive correlations between heart and lipoprotein(a),liver and smoking history,blood deficiency and D-dimer,yin deficiency and hypertension,phlegm and hypertension and Low-density lipoprotein,Qi stagnation and gender.Meanwhile,heart and diabetes,there is a negative correlation between wet and activated partial thromboplastin time.It is inferred that the above risk factors can be used as an objective basis for TCM syndrome differentiation of angina pectoris.
