Study On The Correlation Between TCM Symptoms And The Shape Of Foot Arch Of Qi Stagnation And Blood Stasis Syndrome In Patients With Painful Heel Syndrome

Objective:Through the observation and analysis of the TCM syndrome of qi stagnation and blood stasis type of painful heel syndrome and the related data between it and the shape of foot arch,this paper discusses the correlation between them,so as to provide some references for clinical diagnosis and treatment of qi stagnation and blood stasis type of painful heel syndrome.Methods:From December 2019 to December 2020,58 patients with unilateral painful heel syndrome of qi stagnation and blood stasis were selected from the Department of Orthopaedics and Traumatology,the third affiliated Hospital of Yunnan University of traditional Chinese Medicine.On the basis of "traditional Chinese Medicine combined with Orthopaedics and Traumatology",the main symptoms and secondary syndromes of painful heel syndrome of qi stagnation and blood stasis were screened out,and the corresponding scores were given according to their severity,in which the scores of the main symptoms were0,3,6,9 respectively,and the scores of the secondary symptoms were 0,1,2,3 respectively.The tongue and pulse images of the patients were for dialectical reference only and were not included in the score.As a result,the scoring table of TCM syndrome was established.The patients with calcaneal pain of qi stagnation and blood stasis type were scored quantitatively of TCM syndrome,and the data of VAS score and AOFAS score were collected.At the same time,the patients with heel pain of qi stagnation and blood stasis were tested by plantar pressure test,and the arch index AI of the affected side was collected.After the data collection is complete,the relationship between TCM syndrome score of qi stagnation and blood stasis with painful heel syndrome and foot arch index AI value is analyzed by establishing linear model and nonlinear model.Linear model analysis: linear correlation analysis method is used.If the two samples meet the normality test,Pearson correlation test is used.If the two samples do not meet the normality test,Spearman correlation test is used.Nonlinear model analysis: the curve fitting analysis method is used to determine whether there is a nonlinear relationship between the two samples by observing the significance of the square coefficient of the dependent variable.Finally,through the production of scatter diagram to directly judge the advantages and disadvantages of the relevant model and explore the relationship between TCM syndrome and foot arch shape of qi stagnation and blood stasis syndrome.All of the above data are processed using SPSS22.Results:1.The average score of TCM syndrome of 58 cases of unilateral qi stagnation and blood stasis heel pain was 12.6 ±4.0,including 9 cases of mild,38 cases of moderate and 11 cases of severe.2.The average AI of arch index was 16.00(11.33).The foot type of patients was judged according to the AI value of arch index.Among them,the foot type of patients with this disease was high arch foot in 41 cases(70.7%),normal foot in 3 cases(5.2%),and flat foot in 14 cases(24.1%).3.Linear model analysis showed that there was a negative correlation between arch index AI and TCM syndrome score(r=0.603,P < 0.05),a negative correlation between TCM syndrome score and VAS score(r = 0.423,P < 0.05),and a positive correlation between AI and AOFAS score(r = 0.539,P < 0.05),indicating that the linear model was established.Nonlinear model analysis: there was a U-shaped relationship between arch index AI and TCM syndrome score(β = 0.024,t=6.019,P<0.01),a U-shaped relationship between AI and VAS score(β=0.007,t=4.835,P<0.01),and an inverted U-shaped relationship between AI and AOFAS score(β =-0.073,4.546,P < 0.01),indicating that the nonlinear model is valid.The nonlinear model fitting is better according to the scatter plot.The results showed that there was a U-shaped relationship between the shape of foot arch and the TCM syndrome of patients with painful heel syndrome caused by qi stagnation and blood stasis,as well as the severity of foot pain and functional limitation.Conclusion:1.There is a "U" relationship between TCM syndrome and foot arch shape of qi stagnation and blood stasis type of painful heel syndrome,that is,the high or low shape of foot arch is closely related to the severity of TCM syndrome,and the degree of TCM syndrome of patients with normal foot arch shape is lighter.2.In this group of patients with painful heel syndrome caused by qi stagnation and blood stasis,the proportion of people with high arch foot is the highest,and the higher the height of foot arch is,the more serious the TCM syndrome is.
