Clinical Observation Of Moxibustion Of Ginger And Herbs On Lower Abdomen Combined With Mirena In The Treatment Of Adenomyosis Due To Cold Coagulation And Biood Stasis

Purpose: In this study,moxibustion of ginger and herbs on lower abdomen combined with mirena was used to the treatment of adenomyosis due to cold coagulation and blood Stasis,through the observation and statistical analysis by the TCM syndrome scores,dysmenorrhea degree scores,uterine volume and CA125 change of the curative effect before and after treatment,scientific evaluation of its clinical efficacy and safety,to provide a more effective new program for patients with adenomyosis,and to promote the further popularization and application of integrated Chinese and western medicine in clinical practice.Methods: Patients in the outpatient department of gynecology and inpatient department of Shijiazhuang city hospital of traditional Chinese medicine were selected from September 2018 to December 2019,sixty patients with adenomyosis(cold coagulation and blood stasis)were randomly divided into the treatment group and the control group,with 30 patients in each group.Control group: Mirena was placed only in the uterine cavity on the 3～7 day of the menstrual cycle.Treatment group: On the basis of placement of mirena in the uterine cavity,add moxibustion of ginger and herbs on lower abdomen,every 3 days,period is not stopped.A total of 3 menstrual cycles were observed.Changes in the TCM syndrome scores,dysmenorrhea degree scores,uterine volume and CA125 value were recored before and after treatment,and adverse reactions were recorded in time.SPSS 21.0 statistical software was used to conduct statistical analysisof the collected clinical data by means of independent-sample t test、paired t test、rank sum test and chi-square test methods.Results:1.Comparison of TCM syndrome scores: Compare with in groups:After treatment,the scores of TCM syndrome in both groups decreased,and the difference is statistically significant(p<0.05);Compare among groups: After treatment,The treatment group is superior to the control group,and the difference is statistically significant(p<0.05).The total effective rate of the treatment group is 83.3%,in the control group is 56.7%.Comparison between the two group,the treatment group is significantly better than the control group,and the difference is statistically significant(p<0.05).2.Comparison of dysmenorrhea degree scores: Compare with in groups: After treatment,the scores of dysmenorrhea degree in both groups decreased,and the difference is statistically significant(p<0.05);Compare among groups: After treatment,the treatment group is superior to the control group,and the difference is statistically significant(p<0.05).The total effective rate of the treatment group is 86.7%,in the control group is56.7%.Comparison between the two group,the treatment group is significantly better than the control group,and the difference is statistically significant(p<0.05).3.Comparison of uterine volume size: Compare with in groups: After treatment,the uterine volume in both groups decreased,and the difference is statistically significant(p<0.05);Compare among groups: There is no statistically significant difference between the two groups after treatment(p>0.05).4.Comparison of CA125 value: Compare with in groups: After treatment,the CA125 value in both groups decreased,and the difference is statistically significant(p<0.05);Compare among groups: There is nostatistically significant difference between the two groups after treatment(p>0.05).5.Adverse reactions are counted at the end of treatment: two cases of irregular vaginal bleeding and one case of lumbar acid occurred in the treatment group;in the control group,three cases of irregular vaginal bleeding,two cases of lumbar acid and one case of acne occurred.Conclusions: Moxibustion of ginger and herbs on lower abdomen combined with mirena has more advantages than mirena alone in improving the clinical symptoms and relieving dysmenorrhea in patients with cold coagulation and blood stasis adenomyosis;in terms of reduction of uterine volume and CA125 value,the efficacy is comparable to that of mirena along;there are fewer cases of adverse reactions than those treated with mirena alone.This method has significant clinical efficacy,good safety and is worthy of wide clinical application.
