| Objective:To observe and analyze the intervention effect of Cangfu Daotan decoction on the expression of Visfatin,free fatty acid(FFA)and Chemerin in serum and follicular fluid of infertile patients with phlegm-dampness polycystic ovary syndrome,as well as its effect on oocyte quality,ovulation process and embryonic development potential,trying to proceed from the perspective of lipid metabolism,to explore the mechanism of Cangfu Daotan decoction in improving clinical symptoms and signs,regulating ovulation function and improving pregnancy outcome of in vitro fertilization and embryo transfer(IVF-ET)in patients with phlegm-dampness polycystic ovary syndrome.Methods:A total of 76 patients with polycystic ovary syndrome(pcos)who were diagnosed as phlegm-dampness type(IVF-ET)by traditional Chinese medicine.According to the random number table method,they were averaged divided into experimental group(cangfu daotan decoction+IVF)and observation group(placebo+IVF).Another 38 patients who underwent assisted reproduction due to fallopian tube factors(placebo+IVF)were selected as the control group.All the patients in the three groups were treated with GnRH-a+Daying-35 double reducing length regimen.The changes of phlegm and dampness syndrome scores before and after treatment were compared between the experimental group and the observation group.And the levels of serum endocrine,Visfatin,FFA and Chemerin in serum and follicular fluid were measured.The levels of blood E2,LH and P on the day of HCG injection were calculated,and the body mass index(BMI),waist-to-hip ratio(WHR)fertilization rate and high quality embryo rate were calculated.Results:1.TCM syndrome score:after treatment,the symptoms of phlegm-dampness were significantly improved and the syndrome score decreased significantly in the experimental group;compared with the observation group,there was a significant difference in the score of phlegm-dampness syndrome before and after treatment(P<0.01).2.After treatment,BMI and WHR in the experimental group were significantly lower than before treatment,with statistically significant differences(P<0.01).There was no significant change in BMI and WHR in the observation group before and after treatment,and there was no statistical significance(P>0.05).3.Comparison of fertilization rate and excellent embryo rate among the three groups:the fertilization rate and excellent embryo rate of the control group were higher than those of the experimental group and the observation group,and the difference was statistically significant(P<0.05).Compared with the observation group,the fertilization rate and excellent embryo rate of the experimental group was higher,which was closer to that of the control group(P<0.05).4.There were differences in the expression levels of serum Visfatin,FFA and Chemerin among the three groups:except for serum Chemerin,the levels in the control group were lower than those in the observation group and the experimental group(P<0.01);There was no significant difference in serum Chemerin between the control group and the experimental group(P>0.05).The levels of the three factors in the experimental group were significantly lower than those in the observation group(P<0.05),and the experimental group was closer to the control group.5.There were differences in the expression levels of Visfatin,FFA and Chemerin in follicular fluid among the three groups:the levels of follicular fluid in the control group were significantly lower than those in the observation group and the experimental group(P<0.05);The content of the experimental group was lower than that of the observation group,which was closer to that of the control group(P<0.05).Conclusion:1.Cangfu daotan decoction can improve the symptoms of phlegm and dampness in PCOS patients and regulate the obesity constitution.2.The application of Cangfu daotan decoction can improve the fertilization rate and excellent embryo rate of PCOS patients treated with IVF-ET,suggesting that it may promote the normal development of oocytes and improve the developmental potential of early embryos to some extent.3.Cangfu daotan decoction may improve lipid metabolism by reducing the expression levels of Visfatin,FFA and Chemerin,improve "ovary storage and excretion"function in patients with PCOS,provide sufficient energy supply for oocytes,thus improve egg quality. |
