Study On Screening The Biomarkers Of Phlegm-Dampness Syndrome In Polycystic Ovary Syndrome Based On MicroRNA Expression

Objective: To detect the expression of microRNA-183/200/223 in serum,follicular fluid and granulosa cells of patients with phlegm-dampness syndrome in polycystic ovary syndrome(PCOS),to screen the biomarkers of PCOS phlegm-dampness syndrome and to provide a theoretical basis for its standardization in TCM.Method: During the period from January 2018 to January 2019,female patients with infertility between the ages of 20 and 37 were treated with IVF or ICSI in the Affiliated Hospital of Shandong University of Traditional Chinese Medicine.Sixty infertile patients were divided into PCOS phlegm-dampness group,PCOS non-dampness group,and control group,20 cases in each group.The expression levels of miR-183/200/223 in serum,follicular fluid and granulosa cells of three groups were detected by qRT-PCR,and the relationship between them and phlegm-dampness syndrome was evaluated by correlation analysis.Results: LH,T,LH/FSH,AMH in PCOS phlegm-dampness groups and PCOS non-dampness groups were higher than those in control group(P<0.05).Compared PCOS phlegm-dampness group with non-dampness group,there was no significant difference in T,LH/FSH and AMH.The BMI,FPG,FINS and HOME-IR in the PCOS phlegm group were higher than those in the non-dampness group and the control group(P<0.05).In serum and follicular fluid,the relative level of miR-223-3p in PCOS phlegm-dampling group was higher than that in PCOS non-dampness group and control group,the difference was statistically significant(p<0.05),and both were positively correlated with HOMA-IR.In granulosa cells,the expression of miR-183-5p in PCOS phlegm group was higher than that in PCOS non-dampness group and control group,the difference was statistically significant(p<0.05),and is positively correlated with HOMA-IR(r=0.732,p= 0.007).The expression of miR-200b-3p in the PCOS phlegm group and the PCOS non-dampness group was lower than that in the normal group,but the expression difference between the three groups was not statistically significant.The relative expression of miR-200c-3p in the PCOS phlegm group was higher than that in the PCOS non-dampness group and the normal group,the difference was statistically significant(p<0.05)and positive related with HOMA-IR(r=0.704,p=0.016).The relative expression of miR-223-3p in the PCOS phlegm group and the PCOS non-dampness group was higher than that in the normal group,the difference was statistically significant(p<0.05),and it has positive correlation with HOMA-IR(r=0.547,p=0.042).The expression of miR-223-5p in the PCOS phlegm group and the PCOS non-dampness group was significantly lower than that in the normal group,the difference was statistically significant(p<0.05),and is negative correlated with HOMA-IR(r=-0.671,p=0.024).Conclusion: PCOS phlegm-dampness syndrome has obvious insulin resistance(IR)characteristics,and microRNA expression in PCOS phlegm-dampness syndrome is significantly different.Mi R-183-5p and miR-200c-3p in granulosa cells of PCOS phlegm-dampness group is up-regulated,and its high expression is related to IR,and miR-200c-3p has no significant difference in serum and follicular fluid expression,so miRNA-183-5p is expected to be a specific biomarker for the diagnosis of PCOS phlegm-dampness,miR-200c-3p is expected to be a specific biomarker for ovarian localization.Mi R-223-3p was up-regulated in serum,follicular fluid and granulosa cells of PCOS phlegm-dampness group,and positively correlated with IR.It can be used as a biomarker for screening PCOS phlegm-dampness.The expression of miR-223-5p in PCOS phlegm-dampness group and PCOS non-dampness group was significantly lower than that in control group,and it was negatively correlated with HOMA-IR in PCOS phlegm-dampness group.It could also be used as a biomarker for screening PCOS phlegm-dampness syndrome.
