| Objective:1.To evaluate the clinical efficacy of Banxia Baizhu Tianma Decoction on the syndrome of hypertension phlegm dampness,in order to study the pathological mechanism of hypertension syndrome of phlegm and dampness from the perspective of prescriptions and the target of Banxia Baizhu Tianma Decoction Point study provides clinical evidence;2.The microRNA gene chip technology was used to explore the difference in plasma microRNA expression between patients with hypertensive phlegm-dampness syndrome and normal control group,to screen the differentially expressed microRNA,and to conduct target gene prediction and bioinformatics analysis on it,and to provide a potential target for the intervention study of Banxia Baizhu Ti anma Decoction in the treatment of hypertensive phl egm and dampness syndrome.Methods:(1)Clinical observation:Collect 64 cases of patients with high blood pressure and phlegm dampness syndrome,and randomly divide them into a treatment group and a control group,including 32 cases in the treatment group and 32 cases in the control group.The treatment plan is the conventional Western medicine treatment of the control group,and the treatment group is given the addition and subtraction treatment of the Chinese medicine Banxia Baizhu Tianma Decoction.The effects of Banxia Baizhu Tianma Decoction on hypertensive patients were observed from the aspects of ordinary blood pressure,ambulatory blood pressure,blood lipid level and the score of TCM symptoms.(2)Experimental research:The plasma of 12 volunteers with high blood pressure and phlegm dampness syndrome and the plasma of 12 healthy volunteers were enrolled in the outpatient department and ward of the cardiology department of Shandong Provincial Hospital of Traditional Chinese Medicine.Sending the obtained plasma samples to Shanghai Zhongke Purui Gene Technology Co.,Ltd.Using Agilent Human miRNA high-throughput gene chip for microRNA detection,screening differential microRNA,target gene prediction,functional enrichment analysis of differentially expressed microRNA,and using qRT-PCR method to verify microRNA sequencing of high-throughput chip to explore microRNA expression Differences and significance,summarizing the microRNA gene expression characteristics of hypertensive phlegm dampness syndrome.Results:(一)Clinical observation:(1)In terms of disease efficacy,the total effective rate of disease efficacy in the treatment group was 83.87%,and that in the control group was 62.06%.The efficacy of the treatment group was better than that of the control group(P<0.05).(2)Comparisons of common blood pressure after treatment,the blood pressure levels of the two groups were effectively reduced,and the treatment group was better than the control group(P<0.05).(3)Comparison of ambulatory blood pressure at 24h after treatment,the indicators of ambulatory blood pressure at 24h in both groups were lower than before treatment,the difference was statistically significant(P<0.05);comparison between groups,the average systolic blood pressure at 24h and the average diastolic blood pressure at 24h between the two groups There are differences in the average systolic blood pressure during the day and the decrease in the average systolic blood pressure at night(P<0.05),the therapeutic effect of the treatment group is better than the control group;there is no statistically significant difference in the average diastolic blood pressure during the day and the average diastolic blood pressure at night(P>0.05).(4)In terms of blood lipid levels,after treatment,total cholesterol and low-density lipoprotein in the treatment group were signif icantly lower than before(P<0.05),and there was no significant change in the control group(P>0.05);compared between the two groups,the two groups had differences in the reduction of total cholesterol and low density lipoprotein(P<0.05),the treatment group was better than the control group(5)Compared with TCM symptom scores,the improvement of TCM syndrome scores in the treatment group was better than that in the control group(P<0.05).(6)Comparison of individual TCM symptom scores,after treatment,the treatment group had an improvement effect on all individual symptoms(P<0.05);the control group had an improvement effect on dizziness,head bloating,headache,head weight,chest numbness,nausea,vomiting and sputum saliva(P<0.05);but no improvement on sleepiness,anxiety,insomnia,palpitations,etc.(P>0.05);comparison between the two groups,the results suggest that the treatment group is better than the control group in improving symptoms of TCM,the difference is statistically significant(P<0.05).(二)Experimental research:(1)Hypertensive phlegm dampness syndrome patients and healthy control group were screened out of 10 differentially expressed microRNAs,including 3 up-regulated microRNAs(hsa-miR-16-5p,hsa-miR-197-5p,hsa-miR-642a-3p)and 7 down-regulated microRNAs(hsa-miR-1296-3p,hsa-mi R-3613-3p,hsa-miR-4324,hsa-mi.R-4485-3p,hsa-miR-6090,hsa-miR-6741-3p,hsa-miR-7975).(2)Functional analysis of the differentially expressed microRNA target genes,GO analysis showed that the target genes were mostly enriched in vascular endothelial functi on,axis extension in somatic cell development,arterial endothelial differentiation(biological process),and CRD-mediated mRNA Stable complex,protein phosphatase type 1 complex,RISC complex(cell component)and connexin connexin binding,serine/threonine phosphatase inhibition,I-SMAD binding(molecular function),KEGG analysis showed that the target gene is rich Collected in Wnt signaling pathway,Hippo signaling pathway,Foxo signaling pathway.Conclusion:Banxia Baizhu Tianma Decoction has a certain clinical effect on the treatment of hypertensive phlegm and dampness syndrome.The effect is better than the western medicine treatment group,which can effectively reduce the blood pressure,total cholesterol and low density lipoprotein levels and improve clinical symptoms.Compared with healthy people,the plasma microRNA expression profile of patients with hypertensive phlegm-dampness syndrome is significantly changed.Hsa-miR-16-5p and hsa-miR-4324 may be used as potential markers of hypertension phlegm-dampness syndrome,biological information analysis shows that the differentially expressed microRNA is related to the occurrence of hypertension,metabolic diseases and tumors.The pathogenesis involved may be related to the Wnt signaling pathway,Hippo signaling pathway and Foxo signaling pathway. |
