Clinical Study Of Huazhuoyisui Decoction On The Recovery Of Neurological Function In Ischemic Stroke

Objective:To observe the recovery of nerve function of huazhuoyisui Decoction in the treatment of acute stage of ischemic stroke patients,to evaluate the safety and clinical efficacy of its formulation,and to explore its internal mechanism according to the change of clinical indicators,so as to provide more theoretical basis for huazhuoyisui Decoction in the treatment of acute stage of ischemic stroke.Methods:From January 1,2019 to December 1,2019,72 patients who met the inclusion criteria in the Department of Neurology and cadre health care of the Second Affiliated Hospital of Shandong University of traditional Chinese medicine were collected.The patients included in the study were divided into the control group and the treatment group by random number table method.The control group was given basic medical treatment such as lipid-lowering,antiplatelet aggregation,anticoagulant and neuroprotective agent.The treatment group was given huazhuoyisui formula on the basis of routine treatment in the control group.The treatment time was 14 days.NIHSS score,ADL(Barthel Index)score and TCM syndrome score were assessed before treatment,14 days in group and 90 days in group respectively;Mrs grade was assessed before treatment and 90 days in group;safety index and lipid index were measured before treatment and 14 days in group;recurrence and adverse events were recorded in 90 days in observation period.Results:72 cases were included in the clinical observation(36 cases in the observation group and 36 cases in the control group),3 cases in the treatment group and 2 cases in the control group.67 cases completed the experiment,with a drop rate of 6.9%.There was no significant difference between the two groups in gender,age,basic medical history,etc.before treatment,there was no significant difference between the two groups in TCM syndrome score,neurological deficit score,ADL score and Mrs grade evaluation,which was comparable.(1)Efficacy index comparison:three months after entering the group,the total score of TCM syndromes,neurological deficit score,daily life activity score in the two groups and the comparison betwreen the groups are statistically significant,and the treatment group is superior to the control group;the treatment group is superior to the control group in the comparison of TCM syndrome efficacy and Western medicine clinical efficacy;the comparison of Mrs grade shows that there is no comparison between the control group and the control group after entering the group for three months There was statistical significance between the two groups.The treatment group was better than the control group.(2)Comparison of blood lipid indexes:there was no significant difference in the total cholesterol content among the groups after 14 days in the group.There was statistical significance in the comparison of triglycerides,low-density lipoprotein cholesterol and high-density lipoprotein cholesterol among the groups.The treatment group was better than the control group.(3)Inflammatory index hs CRP comparison:14 days after entering the group,the value was significantly lower,the two groups were compared within the group and between the groups were statistically significant,and the treatment group was better than the control group.(4)Safety evaluation:during the treatment,1 patient in the treatment group had epigastral discomfort,1 patient felt bitter in the mouth,and 1 patient in the control group had diarrhea and abdominal discomfort,but all the symptoms were mild.After 2-3 days,the symptoms were relieved and the drug was not stopped.Before and after treatment,the blood routine,stool routine,liver and kidney function,ECG and other indexes of the two groups were observed,and no obvious abnormality was found,and both treatment methods were safe.Conclusion:(1)Huazhuoyisui formula combined with conventional western medicine treatment scheme is superior to the control group in TCM syndrome score and TCM syndrome effective rate.(2)The therapeutic effect of huazhuoyisui formula combined with western medicine on NIHSS score,ADL score and Mrs score of the patients was better than that of the control group.(3)Huazhuoyisui decoction combined with conventional western medicine and conventional western medicine alone can effectively improve the level of blood lipid and HC CRP,but the adjustment level of huazhuoyisui decoction combined with conventional western medicine is better,the difference is statistically significant.(4)During the treatment and observation period,there was no obvious adverse reaction in the two groups.The safety blood indexes were obviously abnormal.Both treatment schemes were safe.In conclusion,the combination of huazhuoyisui formula and conventional western medicine treatment plan has good clinical effect on patients with acute ischemic stroke,which is more efficient than the conventional western medicine treatment plan,and can effectively improve the neurological function score of patients and regulate the blood lipid level of patients,reduce the inflammatory response,with safety,so as to further promote the clinical application.
