| ObjectiveConstipation is one of the common complications after stroke, which seriously affects the quality of life and prognosis of stroke patients. The most commonly used treatments are oral laxatives and enemas, which can temporarily relieve patients’ symptoms, but easily form the habit of dependence and increase the risk of diarrhea. There are many traditional Chinese medicine treatments for constipation, but the effect and maintenance time of them are different. Therefore,it is necessary to explore more effective and practical treatments. In this research,by evaluating the therapeutic effect of“Air Entraining Guiyuan”by mox ibustion and comparing it with conventional acupuncture, the therapeutic effect of the two groups was objectively analyzed,so as to provide a reliable and applicative scheme for clinical treatment.MethodsFrom March 2019 to December 2019,62 patients with "Qi" deficiency type constipation after stroke were collected from the acupuncture and encephalopathy clinic of the first affiliated hospital of the Guangzhou University of TCM and the Guangdong hospital of TCM.They were randomly divide into the observation group and control group for 31 patients each. Both groups received routine acupuncture with Tianshu(ST 25),Shangjuxu(ST 37),Zhigou(TE 6),Zhaohai(KI 6).On the basis of routine acupuncture, the observation group received mild moxibustion to“Air Entraining Guiyuan”of Zhongwan(CV 12),Xiawuan(CV 10),Qihai(CV 6),Guanyuan(CV 4)and the control group received mild moxibustion to Tianshu(ST 25),Shangjuxu(ST 37).Patients in both groups were treated once a day,5 days a week, for a total of 2 weeks. The scores of constipation clinical symptoms and patient-assessment of constipation quality of life(PAC-QOL)were recorded before and 2 weeks after treatment, and the data were analyzed by SPSS 22.0 statistical software.ResultsThere was no significant difference in gender, age, course of disease, primary disease and other general clinical data between the two groups(P>0.05),indicating that the baseline conditions of the two groups were consistent and comparable. Before treatment, the total score of constipation symptoms and PAC-QOL score of between two groups are meaningless(P>0.05),indicating that the two groups of patients were comparable. After two weeks of treatment, the scores of each constipation symptom, total scores of constipation symptoms and PAG-QOL scores of the patients in the two groups were all reduced with statistical significance(P<0.01),suggesting that the constipation symptoms of the patients in the two groups were improved after treatment, and both groups had certain curative effects.Among the constipation symptoms, the defecation speed, defecation intention and accompanied symptom showed of two group showed statistically significant difference(P<0.05).The defecation time, fecal character and defecation difficulty of both groups had no significant difference(P>0.05).The difference of first defecation time between two groups showed significant difference(P<0.01).The difference of PAC-QOL score after treatment and the difference score before and after treatment between two groups showed statistically significant difference(P<0.05).The posttreatment scores of constipation symptoms were significantly different between two groups(P<0.01).The difference of constipation symptom scores between two groups before and after treatment was statistically significant(P<0.05),indicating that the treatment in the observation group was more effective in improving the symptoms of "Qi" deficiency constipation after stroke. In the overall efficacy evaluation, the observation group was 90.32%,which is higher than the control group(80.65%).By rank-sum test, the difference in efficacy between two groups showed statistically significant difference(P<0.05),suggesting that both treatments could improve symptoms of“Qi”deficiency constipation after stroke, and the overall efficacy of the observation group was better than that of the control group.ConclusionAccording to the results of this experiment, the combination of mild moxibustion on“Air Entraining Guiyuan”and acupuncture therapy can improve"Qi" deficiency type constipation and life quality after stroke to a certain extent as same as conventional acupuncture. However, the efficacy of mild moxibustionon on“Air Entraining Guiyuan”combined with acupuncture is better than that of conventional acupuncture, especially in the improvement of defecation, efecation and“Qi”deficiency. This method has advantages in effect, safety, less adverse reactions and simple operation, which is worthy of further promotion and application in future clinical practice. |
