Clinical Observation On Treatment Of Cancer-related Fatigue With Spleen And Kidney Deficiency After Chemotherapy Of Lung Cancer With Buyuan No.1 And Umbilical Cord Therapy

Objective:By observing the clinical effect of Buyuan No.1 Decoction combined with umbilical cord therapy on cancer-related fatigue(spleen and kidney deficiency type)after chemotherapy of lung cancer,to explore whether the combined therapy has synergistic effect,to determine the curative effect,and to provide effective treatment measures for cancer-related fatigue in combination with traditional Chinese medicine.Methods:Ninety patients who met the inclusion criteria were randomly divided into control group(routine treatment),traditional Chinese medicine group(routine treatment plus oral administration of Buyuan Formula 1),combined group(routine treatment + umbilical therapy + oral administration of Buyuan Formula 1),30 cases in each group.All three groups were given standard platinum-containing two-drug chemotherapy.The control group was given basic treatment according to the general condition of the patients.During the observation period,the patients were instructed to work and rest reasonably,exercise properly and give symptomatic treatment such as pain relief,phlegm elimination and vomiting relief when necessary.Chinese herbal medicine group: Buyuan No.1 prescription was decocted into 300 ml water every day.It was taken separately in the morning and evening,starting from the first day of chemotherapy,for 14 consecutive days.Combined group: On the basis of traditional Chinese medicine group,drug was applied to umbilical cord from the first day of chemotherapy,and was replaced every 24 hours for 14 consecutive days.The PIPER fatigue scale score,KPS score,TCM syndrome score,peripheral blood picture,immune index and general safety index were detected one day before chemotherapy and fifteen days after chemotherapy respectively,and adverse reactions were monitored.Statistical software was used for data analysis.Results: Comparison of results before and after treatment:(1)Piper score,There was no significant difference in total fatigue scores and dimensions between the three groups before treatment(P > 0.05).After treatment,the total score of fatigue,behavior,emotion and body dimension of the three groups were P < 0.05,and the difference was statistically significant.(2)KPS score: Before treatment,KPS score of the three groups was compared by variance analysis(P > 0.05),with no significant difference.After treatment,KPS scores of the three groups were compared by ANOVA(P < 0.05),and the difference was statistically significant.Further,the combined group was compared with the traditional Chinese medicine group and the control group,the difference was statistically significant(P < 0.05).(3)In the aspect of TCM syndrome integral:there were significant differences in the improvement of TCM clinical symptoms between TCM group and combined group after treatment(P < 0.05).(4)In terms of peripheral hemogram:there was no significant difference in the comparison of peripheral hemogram before treatment by ANOVA(P > 0.05).After treatment,the white blood cell and platelet values among the three groups were analyzed by variance analysis(P > 0.05),with no significant difference.After treatment,the hemoglobin values of the three groups were compared(P<0.05),and the difference was statistically significant.(5)The level of T lymphocyte subsets in the combined group was significantly higher than that in the traditional Chinese medicine group and the control group(P < 0.05),but there was no significant difference in the CD8 + group.Conclusion:Buyuan 1 formula combined with umbilical cord therapy can effectively improve the fatigue symptoms and quality of life of patients with cancer-related fatigue(spleen and kidney deficiency type)after chemotherapy of lung cancer.It is better than using umbilical cord therapy alone or taking Buyuan 1 formula orally.The combined treatment scheme is safe and effective,and worthy of clinical promotion.
