Objectives To assess the psychological status of adult orthodontic patients wearing ceramic appliance,Invisalign and lingual fixed appliance in early treatment;To compare the difference of the psychological status between groups in different stages of treatment as well as the change with time and to analysis the relevant influencing factors;To assess the difference of gender,age,degree of profession consideration when choosing appliance,personality characteristics,differences in aesthetic satisfaction and psychological adaptation period between groups.To help clinicians understand the psychological state of adult patients with different aesthetic appeal better;To help clinician to develop a reasonable treatment programs;Clinician should to communication and offer psychological guidance in time so as to improve diagnosis and treatment satisfaction.Methods The sample consisted of 88 adult malocclusion patients from Dental Clinic Affiliated to Zhejiang Chinese Medical University,Hangzhou Smile&Art Dental Clinics and Hangzhou Stomatology Hospital from 2014,12 to 2015.10.The patients are comprised three groups:30 ceramic,27 lingual,and 31 Invisalign.Before appliance insertion(T0),patients completed the basic information questionnaire,Eysenck Personality Questionnaire,psychosocial impact of dental aesthetics questionnaire.After appliance insertion,patients are asked to complete the PIDAQ on day 7(T1),1month(T2),3month(T3).After appliance insertion 3 month,the aesthetic satisfaction of the appliance and the psychological adaptation period are assessed.Then assess and analyze the contents of the questionnaire.Results 1.The lingual group are more cared about the society impact and are less confident on the teeth appearance before treatment.They have more orthodontic treatment need.2.In the early treatment(T1),the degree of social influence increased in the ceramic group,then gradually reduced in a month and further improved over time(P<0.05).In the group of Invisalign and lingual,the degree of social influence has no change at the beginning (T1)and decreased with the improvement of the appearance of teeth.3.The aesthetic attitude and teeth self-confidence are improved after starting the treatment in three groups and further improved with the leveling of the dentition(P<0.05),patients’ expectations after the insertion of the appliance affect the aesthetic attitude.4.Patients with neuroticism on the high side are more sensitive to the social impact before treatment;patients wearing transparent ceramic appliance with high neuroticism are more sensitive on the impact of society at the early treatment(T1).5.In the group of Invisalign,the males get higher scores than females at the aspect of aesthetic attitude in early treatment(P<0.05);the one with more professional consideration also get more scores in aesthetic attitude(P<0.05);the older the lower scores in self-confidence on the teeth appearance(P<0.05).6.The adult patients in lingual and Invisalign group are more satisfied on appliance appearance;the Invisalign group adapt to the appliance appearance in shortest time(P<0.05).Conclusion1.Older adult female patients and the one who are more sensitive on the social commentary and less confident on the teeth appearance are more likely to chose the the improved esthetics appliance.Orthodontists should consider it when making a treatment plan.2.Patients wearing buccal appliance especially the ones with high neuroticism are more sensitive on the impact of society.Wearing more beautiful appliance such as lingual appliance and Invisalign can help to reduce such influence.3.In the group of Invisalign,the older the less confident on the teeth appearance;male patients and the one with more professional consideration have more treatment need.4.Patients wearing Lingual appliance and Invisalign have a great aesthetic experience in early treatment,ceramic appliance is also a good choice.5.Orthodontists should focus on the social psychological status of patients before and during treatment.To make the treatment plan with the patient considering the patient’s personality traits,gender,occupation and other factors.The communication and encouragement should be made in time to improve the treatment satisfaction.
