Effect Of Bushen Huoxue Decoction On Adjacent Vertebral Fractures After OVCF Vertebral Augmentation With Kidney Deficiency And Blood Stasis

Objective : To analyze the effect of Bushen Huoxue Decoction on adjacent vertebral fractures after OVCF of kidney deficiency and blood stasis type.Methods : The subjects were hospitalized in the Department of Orthopaedics and Traumatology,Affiliated People ’ s Hospital of Fujian University of Traditional Chinese Medicine from December 2019 to December 2020.Western medicine was diagnosed as single vertebral body(T10-L4)osteoporotic compression fractures,and 83 patients with kidney deficiency and blood stasis syndrome were diagnosed.The patients were randomly divided into two groups,42 cases in the control group and 41 cases in the experimental group.The control group was given calcium carbonate D3 tablets + calcitriol capsules on the second day after operation,and the experimental group was given Bushen Huoxue decoction on the basis of the control group on the second day after operation.The course of treatment was 3 months.The levels of bone mineral density,bone markers,VAS score,modified ODI dysfunction index and adjacent vertebral fractures within 3 months after operation were observed and recorded in the two groups.The effect was judged after treatment,and the data were statistically analyzed by SPSS20.0.Results :1.A total of 83 patients were included in this study,3 patients fell off,the final control group 40 cases,40 cases in the experimental group.There was no significant difference between the two groups in general before treatment(P > 0.05),indicating comparability.2.Bone mineral density level : The bone mineral density levels at each observation point in the two groups after treatment were significantly improved compared with those before operation.One month after operation,there was no significant difference in bone mineral density between the two groups(P > 0.05).Three months after operation,there was significant difference in bone mineral density between the two groups(P < 0.05).The effect of maintaining bone mineral density in the experimental group was better than that in the control group.3.Level of bone markers : The levels of bone markers at each observation point in the two groups after treatment were significantly improved compared with those before operation.At 1 week and 1 month after operation,there was no significant difference in bone markers between the two groups(P > 0.05).At 3 months after operation,there was significant difference in bone markers between the two groups(P < 0.05).4.VAS score : The VAS scores of each observation point in the two groups after treatment were significantly lower than those before treatment,and the decrease in the experimental group was more obvious with the prolongation of time.One week after treatment,there was no significant difference in VAS scores between the two groups(P >0.05).The VAS scores of the two groups in January,February and March after treatment were compared between the groups at the same period,and the difference was statistically significant(P < 0.05).It indicated that the pain relief in the experimental group was better than that in the control group,and the effect became more obvious with the prolongation of treatment time.5.Improved ODI score : ODI scores of each observation point in the two groups after treatment were significantly lower than those before treatment,and the decrease in the experimental group was more obvious with the prolongation of time.There was no significant difference in ODI score between the two groups 1 week after treatment(P >0.05).The ODI scores of the two groups in January,February and March after treatment were compared between the groups at the same period,and the difference was statistically significant(P < 0.05).It is suggested that the improvement of activity dysfunction in the experimental group is better than that in the control group.With the increase of treatment course,the recovery of functional activity is more ideal.6.Postoperative adjacent vertebral fractures : After 3 months of follow-up,the refractures of adjacent vertebral bodies in the two groups were compared,2 = 2.003,P = 0.157(P > 0.05),with no statistical difference.Conclusion : Bushen Huoxue Decoction is helpful to maintain the bone mineral density level,improve bone metabolism,improve postoperative quality of life and relieve pain in patients with OVCF of kidney deficiency and blood stasis type,thereby reducing the risk of adjacent vertebral fractures,and no obvious adverse reactions are found during the treatment.
