Objective: To observe the clinical efficacy of Tongdu Huoxue Decoction,radiofrequency neurolysis of posterior spinal nerve under intervertebral foramen endoscope and their combination in the treatment of posterior lumbar spinal nerve syndrome,and analyze the advantages and disadvantages of each treatment scheme,so as to provide clinical basis for the promotion of Tongdu Huoxue Decoction Combined with radiofrequency neurolysis of posterior spinal nerve under intervertebral foramen endoscope.Methods: from November 2018 to July 2020,90 patients who met the diagnostic criteria,inclusion criteria and case exclusion criteria of lumbar spinal nerve posterior branch syndrome were randomly divided into Tongdu Huoxue Decoction group,transforaminal endoscopic radiofrequency neurolysis group and combined group according to different treatment methods The Tongdu Huoxue Decoction group was treated with oral Tongdu Huoxue Decoction,200 ml each time,twice a day,after breakfast and dinner,a total of 14 doses;the simple radiofrequency release group was treated with radiofrequency release of the posterior branch of lumbar spinal nerve in the operating room after admission.After eliminating the contraindications,the patients were treated with radiofrequency release of the posterior branch of lumbar spinal nerve under C-arm local anesthesia In the combined group,radiofrequency neurolysis of the posterior branch of lumbar spinal nerve was also performed,and Tongdu Huoxue decoction was given orally,200 ml twice a day,a total of 14 doses.Before treatment and 24 hours,5 days,2 weeks and 1 month after treatment,the visual analogue score of patients in each group was evaluated,VAS)and the Oswestry disability index(ODI)scores were recorded.After 3months of treatment,the modified macnab efficacy evaluation table of each group was recorded,compared,observed and analyzed,and the clinical efficacy of three groups of patients after different treatment was elaborated.Results: there was no significant difference in the scores of the three groups before treatment(P > 0.05).The three treatment methods can play a therapeutic role in the treatment of lumbar spinal nerve posterior branch syndrome;the combined treatment is more effective than the single use of Tongdu Huoxue Decoction and the single use of radiofrequency release of lumbar spinal nerve posterior branch under intervertebral foramen microscope(P >0.05);Conclusion: Tongdu Huoxue Decoction Combined with transforaminal endoscopic radiofrequency neurolysis is an effective method for the treatment of posterior ramus of lumbar spinal nerve syndrome,with short treatment cycle,simple and safe operation and short hospital stay.It is an exploration of integrated traditional Chinese and Western medicine in the treatment of posterior ramus of lumbar spinal nerve syndrome,and also an effective method worthy of clinical development and promotion.
