[Purpose] With the rapid development of industrialization and urbanization,the rural population has continuously moved to towns,settled in towns and become "new citizens".Whether the new citizen group enjoys fair basic medical and health services as the original urban residents is the key to measuring whether the new citizens can get a healthy urban life,and it is also a measure of whether the medical and health service system in the urbanization process is balanced and fully developed.Therefore,this study takes Hubei Province as an example to investigate the needs,utilization and satisfaction of the basic medical and health services of new citizens in the process of urbanization.By comparing with the original urban residents’ various levels,discover differences and problems,and explore strategies for improving basic medical and health services in the process of urbanization.In order to provide a scientific basis for improving the quality of urbanization in Hubei Province and building a healthy Hubei.[Methods](1)The combination of theoretical research and empirical analysis.Mainly conduct theoretical research on related concepts,research status,method introduction,research overview,etc.;empirical analysis of the utilization and satisfaction of basic medical and health services for new citizens.(2)Combining quantitative analysis and qualitative analysis.Use SPSS21 statistical software,frequency,effective percentage,chi-square test to quantitatively analyze and compare the basic medical and health service needs,utilization,improvement status,satisfaction,etc.of new citizens and original urban residents;use Stata16 analysis software,adopt propensity score matching method(PSM)quantitatively investigates the impact of the urbanization household registration system reform;use professional talks and literature research methods to qualitatively study the problems and countermeasures in the medical and health field during the urbanization process.(3)Combining comparative analysis and retrospective analysis.Analyze the differences between the new citizens and the original urban residents in various aspects through horizontal comparisons;analyze the changes in the family’s economic life and the improvement of basic medical and health services before and after the settlement of the new citizens through longitudinal review.[Results](1)In terms of family economic life,although the status of family economic life has improved a lot after the new citizens settled,it is still lower than the original urban residents.(2)The new citizens’ personal health knowledge,health behavior,and access to basic medical and health services lag behind those of the original urban residents.(3)The self-reported basic medical service needs of the new citizens are lower than those of the original urban residents,which is mainly manifested as the prevalence of chronic diseases(18.8%)and two-week prevalence(11.4%)of the new citizens is lower than that of the original urban residents;The utilization rate of shortterm outpatient services by new citizens is higher than that of long-term inpatient services,which is mainly manifested in the two-week visit rate(14.0%)of new citizens is higher than the annual hospitalization rate(9.6%).(4)The utilization level of new citizens’ basic public health services is low,mainly manifested as the new citizens’ health file filing rate(16.4%),the frequency of receiving health education,and the signing rate of family doctors(7.4%)are all at a low level,and lower than the original urban residents;although compared with before the settlement,the utilization rate of various public health service projects has improved,but the utilization rate of chronic disease health management,elderly health management and other aspects is still low.(5)The overall satisfaction of the new citizens with the current overall living conditions,medical technology level,medical service costs,ease of medical treatment,medical service attitude,public health services,and medical service system are 59.1%、53.9%、39.1%、55.3%、52.3%、44.5%、48.1%,all lower than the original urban residents’ satisfaction level.(6)The average treatment effect(ATT)of the urbanization household registration system reform on the satisfaction ratings of the new citizens’ public health service and medical service system are 3.4266 and 3.4589,the reform of the urbanization household registration system has a positive impact,which improves the satisfaction score of public health services and medical service systems.[Conclusions] It is recommended to actively promote employment and entrepreneurship so as to improve the economic living standards of new citizens ’families;focus on new citizens,strengthen health education,improve residents’ health literacy,and improve health behavior;by strengthening the construction of grassroots service capabilities,accelerating the promotion of graded diagnosis and treatment,and vigorously developing the Internet + Smart healthcare and other measures to further improve the accessibility of basic medical and health services for new citizens;adapt to social needs,pay attention to special groups,and meet the needs of residents’ medical and health services;accelerate structural reforms on the supply side of medical and health care,complement supply shortcomings;improve public health system to promote the development of healthy cities and towns.
