Research purposes: Study the clinical characteristics of colorecta l polyps,the different distribution of TCM syndromes and the TCM syndromes of colorectal polyps and summarize the TCM syndromes of colorectal polyps to lay the foundation for the follow-up study.Research methods: 1.From August 2018 to March 2019,153 po lyps patients who were confirmed by endoscopy center of Dawn Hospital Affiliated to Shanghai University of Traditional Chinese Medicine and met the inclusion and exclusion criteria were selected to fill in the questionnaire.2.According to the clinical symptoms of patients,combined with the information of the four diagnostic methods,and referring to Syndrome Element Differentiation,the disease location syndrome elements and disease nature syndrome elements were extracted,and then the extracted syndromes were added together.According to the diagnostic criteria of syndromes,the syndromes which are larger than or equal to 20 are selected.According to patient’s age,gender,polyp location,polyp size,endoscopic classification,pathological classificatio n,clinical symptoms,TCM syndromes,TCM constitution database was established.3.Through statistical analysis,the age,sex,location,size,endoscopic classification and pathological classification of colorectal polyps were analyzed.The distribution characteristics of constitution and syndrome elements in TCM were analyzed.The relationship between disease location syndrome elements and disease-related syndrome elements was summarized.Research purposes: To study the clinical characteristics of colorectal polyps and the characteristics of TCM syndromes and their different distribution patterns,summarize the TCM syndromes of colorectal polyps,and lay the foundation for the follow-up study.Research methods: 1.Among 153 patients with colorectal polyps,81 were male and 72 were female;36 were younger than 40 years old,73 were between 40 and 60 years old;44 were older than 60 years old;32 were polyps located in rectum,49 in sigmoid colon,9 in descending colon,40 in transverse colon and 21 in ascending colon;55 in polyps less than 0.5 cm,82 in polyps less than 0.5 cm to 1 cm.There were 16 polyps > 1 cm,90 polyps belonged to type I,23 polyps belonged to type I,13 polyps belonged to type II,27 polyps belonged to type III,46 polyps belonged to proliferative type,5 polyps belonged to adenomatous type,67 tubular adenomas,20 tubular villous adenomas,6 serrated adenomas and 9 mixed polyps.The clinical symptoms of colorectal polyps may be constipation,diarrhea,abdominal pain and other discomforts.2.In 153 polyps patients,there was no difference in the morbidity of male and female patients in different age groups;there was no significant difference in the size of polyps in different parts;there was no significant difference in the shape of po lyps in different intestinal segments;there were differences in the size of endoscopic polyps;there were differences in the location of polyps in different pathologies;there was no significant difference in endoscopic polyps in different pathologies;3.There are four types of constitution in 153 polyps patients: Q i deficiency,Q i depression,phlegm dampness and damp-heat.There are gender differences in patients with Q i depression,mainly in women.The majority of patients with qi-depression and phlegm-dampness were 40-60 years old,and those with Damp-heat were less than 40 years old.There were differences in the size of polyps in patients with Q i deficiency,mainly less than 0.5 cm.There are differences in pathological types of polyps in patients with Qi depression,mainly in proliferative polyps.4.Syndromes related to colorectal polyps include spleen,stomach,liver,kidney and large intestine.There are differences between males and females in patients with liver lesions,and the liver lesions are predominant in females.Spleen and liver were the main sites of disease in patients aged 40-60 years,while kidney was the main site of disease in patients aged over 60 years.There was no difference in the distribution of polyps among patients with different diseases.The location of patients with polyp size ranging from 0.5 cm to 1 cm is related to the large intestine,and that of patients with polyp size less than 0.5 cm is related to the liver.The location of patients with type Is polyps is associated with the large intestine.Patients with tubular adenoma and proliferative polyp are associated with the location of the disease in the large intestine,and patients with hepatic polyp are associated with the location of the disease.Abdominal pain is the main disease in patients with large intestine.The patients with spleen or liver disease may have no obvious constipation,and most patients with kidney disease have constipation.The patients with spleen or liver disease may have no obvious constipation,and most patients with kidney disease have constipation.5.Diseases related to colorectal polyps include Q i deficiency,Yang deficiency,Q i stagnation,internal dampness and damp-heat.Q i stagnation is more common in women than in men.The manifestations of Q i deficiency and Qi stagnation were the main manifestations in patients aged 40-60 years,while those aged over 60 years were mainly manifested as Yang deficiency.There was no difference in the distribution of polyps among patients with different diseases.Polyps less than 0.5 cm were associated with Q i stagnation,while patients with polyps between 0.5 cm and 1 cm were associated with internal dampness and damp-heat.Patients with polyps of type I are associated with damp-heat.The disease belongs to proliferative polyps mainly with Qi stagnation.The patients with tubular adenoma are related to the disease of polyps with internal dampness and damp-heat.Patients with internal dampness and damp-heat may also have no obvious abdominal pain.The patients with Q i deficiency have no obvious constipation,while the patients with Yang deficiency mostly show constipation.Patients with internal dampness and damp-heat may have no obvious diarrhea.6.Cluster analysis shows that there are four types of diseases: 1)the location of the disease is in the spleen,the nature of the disease belongs to Qi deficiency and internal dampness;2)the location of the disease is in the large intestine,the nature of the disease belongs to damp-heat;3)the location of the disease is in the liver,the nature of the disease belongs to Q i stagnation;4)the location of the disease is in the kidney,and the nature of the disease belongs to Yang deficiency;Research conclusions: 1.The size of polyps in patients with colorectal polyps ranged from 0.5 cm to 1 cm,and the type of polyps in endoscopy was mainly type I polyps.Tubular adenoma is the main pathological feature of polyps.2.There are four types of TCM constitution in patients with colorectal polyps: Q i deficiency,Q i depression,phlegm-dampness and damp-heat.There were differences among patients of different ages,polyp size and pathological classification.3.Colorectal polyps are located in the large intestine,involving the liver,spleen,kidney and stomach.Pathological factors include Q i stagnation,internal dampness,damp-heat,Q i deficiency and Yang deficiency.The disease location syndrome elements and disease-nature syndrome elements are different in d ifferent gender,age,size of polyps,endoscopic classification and pathological classification of polyps patients.This experiment can be divided into four syndromes.4.Syndrome differentiation has objectivity and universality,and it is one of the methods suitable for the study of TCM syndromes...
