| Objective:This research in Chinese medicine theory as the instruction,follow the principle of"graded",through the survey and literature research,add common symptoms of acute mastitis,integrate the quantitative standards of symptoms and signs,combing traditional Chinese medicine(TCM)nursing technique content and method of operation,build optimized version of acute mastitis of TCM nursing plan,in order to further standardize the TCM nursing behavior,improve the quality of TCM nursing,as a reference of other TCM nursing plan to improve.Methods:1.A semi-structured interview method:through the early literature read and retrospective analysis to determine the interview outline,interviewed with 16 medical personnel from three hospitals in Beijing,the interview data analysis and induction,understand the acute mastitis of TCM plan of the implementation of the specific situation,analyzed the problems existing in the existing schemes.2.Questionnaire survey method:based on literature reading and combined with the results of previous interviews with medical personnel,a self-developed questionnaire on the disease status of patients with acute mastitis was developed,and quantitative criteria of symptoms and signs were integrated in the development process.The convenience sampling method and the self-made questionnaire were used to investigate the acute mastitis patients from three hospitals in Beijing,to explore the incidence and severity of symptoms and signs,and to understand the application status of clinical nursing techniques of TCM.3.Literature research method:based on the previous questionnaire survey,the retrieval subject was identified,the modern literature related to the purpose of the study was systematically searched,the literature was screened according to the strict criteria,and the contents were extracted and descriptive analysis were conducted to summarize the nursing points.Select and extract the teaching materials related to the research purpose to form the nursing points.4.Expert consensus method:the nursing points were selected and discussed through the combination of the early issuance of expert consultation questionnaire and the later implementation of expert meeting,and the nursing items of the program symptoms and TCM nursing techniques were determined to make the contents and items available.Results:1.Through interviews with 16 medical personnel,six themes were extracted,that is,patients with symptoms not mentioned in the program but obvious,such as simple milk stasis;It is necessary to carry out integral quantification of symptoms;The effectiveness and suitability of TCM nursing techniques should be further demonstrated.The operation method and technique should be further standardized.The nursing effect evaluation lacks the objective index support;“Chinese medicine nursing programs" can be applied to outpatient work.2.Through the questionnaire survey of 176 patients,descriptive analysis was used to analyze the common symptoms and their severity.The incidence rate and the top 4 severity:breast pain,breast swelling,galactostasis,and thirst.Signs the incidence and severity of the former four:lumps formation,reddish or red skin,increased body temperature,cracked nipples;The effective degrees of TCM nursing techniques were:methods row of milk,Externalapplications,auricular acupressure,acupoint massage.3.Through literature research into the methods row of milk with 127 papers,13 papers of breast pain-Externalapplications,2 papers of breast swelling-Externalapplications,22 papers of galactostasis-Externalapplications,14 papers of fever-Externalapplications,24 papers of breast pain-auricular acupressure,3 papers of breast swelling-auricular acupressure,5 papers of galactostasis-auricular acupressure,18 papers of fever-auricular acupressure,18 papers of thirst-auricular acupressure;11 textbooks,then extraction and analysis of the contents,form the nursing key points.4.After the expert consultation,573 nursing points were initially selected,and the draft was formed according to the principle of nursing points of more than half of the patients who were supported by experts.After the discussion,the final nursing points were determined and the contents of TCM nursing plan were formed.Conclusion:In this study,based on the improvement needs of medical personnel and the characteristics of patients’ diseases,the quantitative standards of symptoms and signs were integrated on the basis of the original plan,and the corresponding contents of symptoms and TCM nursing techniques were improved,so as to increase the suitability and standardization of the plan content,and provide reference for the optimization of other TCM nursing plans.However,this study has not carried out the clinical pilot application of the program,and the clinical practicability should be further explored in the later period. |
