| Vitiligo,known as "white barrage" in traditional Chinese medicine,appears clinically as pale white,milky white or porcelain white spots or patches.The shape,size and number of vitiligo are uncertain,which is a primary,localized,or generalized loss of skin and mucous membrane pigment,due to the weakening of the function of tyrosine system in melanocytes of skin and hair follicles.According to epidemiological data,vitiligo can occur in any race and region,and of all ages,but most of them are teenagers.There is no significant difference in the ratio of male and female.The incidence of vitiligo is positively correlated with the depth of skin color.The global prevalence rate is 0.50%～4.0%,the population prevalence rate is no less than 1%～2%,and the prevalence rate in China is 0.1%～2.7%,showing an increasing trend year by year,and vitiligo patients are prone to accompany with lupus erythematosus,psoriasis and hyperthyroidism and other autoimmune diseases.Although the disease is easy to diagnose,but the pathogenic factors are complex,there is no clear and effective treatment.The external treatment of traditional Chinese medicine is simple,effective,less side effects and has unique advantages.In this paper,the combination of traditional Chinese medicine theory and modern medical techniques was used to explore the effect of combined treatment of tincture and narrow-spectrum ultraviolet B,trying to explore a kind of treatment method which is curative,small side effect and suitable for chronic course of disease.Objectives of the study:Through a clinical study on the treatment of stable,localized and deficiency of liver-kidney vitiligo,treating by Baibo tincture combined with narrow-spectrum ultraviolet B,observing the changes of skin lesion area before and after treatment,verifying the clinical efficiency of the combined therapy and compare it with that of narrow-spectrum ultraviolet B irradiation alone,exploring the effectiveness and safety of external use of tincture of traditional Chinese medicine and highlighting the characteristics of traditional Chinese medicine and external treatment for vitiligo.Research methods:From November 2018 to February 2019,60 patients with vitiligo who voluntarily participated in the subject and met the criteria were randomly divided into the treatment group and the control group,with 30 cases in each group.In the treatment group,Baibo tincture was used twice a day and combined with narrow-spectrum ultraviolet B irradiation(the initial dose was 0.3J/cm2,then increased 0.1J/cm2,twice a week,according to the skin lesions).The control group was exposed to narrow-spectrum ultraviolet B alone(the same as the method of treatment group).The course of treatment was 12 weeks.After 4,8,12 weeks,the area of white spot and the area of restoring skin color were calculated,the color of white spot was observed,judging the curative effect,and the traditional Chinese medicine syndromes before and after treatment were graded,then to record whether there were any adverse reactions.At last,SPSS20.0 statistical software was used to analyze the data.Results of the study:A total of 55patients(without 5patients who left out the experienment)with deficiency of live r-kidney,stable phase and localized vitiligo were studied in this study.There were 28 patients in the treatment group and 27 patients in the control group.The data of statistical analysis showed that there was no significant statistical difference in the site of skin lesions,in gender,age and course of disease of the patients in the treatment group and the control group,so it was comparable in clinical practice.After 12 weeks of treatment,the total effective rate of the treatment group was 78.57%and that of the control group was 55.56%.There was significant difference in the total effective rate between the two groups(P<0.05).The curative effect of the treatment group was better than that of the control group.The white spot area of the two groups was comparable before treatment,and decreased after 12 weeks of treatment.The therapeutic effect of the treatment group was more significant than that of the control group,and the white spot area of the treatment group was significantly higher than that of the control group(P<0.05).After 12 weeks of treatment,the scores of each item in the two groups were lower than those before treatment,and the scores in the treatment group were significantly lower than those in the control group.It has statistical significance.During treatment and follow-up,there were no significant adverse reactions between the two groups.Conclusion:Baibo tincture combined with narrow-spectrum ultraviolet B is effective in treating deficiency of liver-kidney,stable-phase and localized vitiligo,which can effectively reduce the area of white spot,decrease the color of it,and improve the scores of traditional Chinese medicine syndromes in the two groups included dizziness,tinnitus,insomnia,acid and soft waist and knee syndromes.The total effective rate of the treatment group is better than that of the narrow-spectrum ultraviolet B irradiation group,and there is no obvious adverse reaction,which is worthy of further promotion in clinical practice. |
