Analysis The Rule Of Medication Used By Director Si Fuquan In Treating Chronic Glomerulonephritis Hematuria With TCM

Objective: By digging the medication rule of Director Si Fuquan for the treatment of chronic glomerulonephritis hematuria and in-depth analyzing his clinical differentiation and treatment according to different syndromes and signs to improve clinical efficacy.Hoping to provide new ideas for the treatment of chronic glomerulonephritis hematuria.Methods: From May 2017 to October 2019,this study collected a total of 60 cases of chronic glomerulonephritis with hematuria and 217 prescriptions.They were all treated in the clinic of Director Si Fuquan.After Standardizing data,establishsd a database accroding to traditional Chinese medicine,four natures,five tastes,channel tropism and efficacy.Then,analyzed the frequency of traditional Chinese medicine and found out the core.Made descriptive statistics of relevant data and got the results to form a chart,then got some new Chinese medicine combinations by importing the core traditional Chinese medicine in the prescription into SPSS24.0 software for cluster analysis.Finally,summarized Director Si Fuquan’s experience and thoughts on treatment of chronic glomerulonephritis hematuria and the rules of medication by analyzing the frequency of four natures,five tastes,channel tropism,efficacy and the new combinations.Results: 1 There were a total of 60 medical records and 217 consultations,including 22 males and 38 females.Among them,37% were male patients and 63% were female patients and the ratio of male to female was 1:1.73.And the oldest was 76 while the youngest was 23.The average age was 47.45 ± 13.36 years old.The majority of patients were young and middle-aged women.Most of the symptoms were asthenia and lumbago.The tongue image is mostly light red tongue,yellow putty and moss,and the pulse image is mostly weak.The deficiency of qi and yin is more common in asthenic syndrome,and the damp heat syndrome is more common in thenic syndrome.2 There were 144 traditional Chinese medicines,23 of which were used more than 50 times,and the order from high to low was radix astragali(217 times),caulis piperis kadsurae(189 times),fructus corni(184 times),rhizoma Chuanxiong(171 times),radix rehmanniae(167 times),caulis sinomenii(163 times),radix cynanchi paniculati(162 times),twig of winged euonymus(161 times),radix achyranthis bidentatae(158 times),rhizoma smilacis glabrae(155 times),fructus ligustri lucidi(143 times),herba ecliptae(134 times),radix boehmeriae(131 Times),rhizoma imperatae(129 times),radix paeoniae alba(112 times),rhizoma cimicifugae(102 times),folium pyrrosiae(99 times),radix rubiae(80 times),semen euryales(72 times),cortex moutan(69 times),fructus rosae laevigatae(69 times),field thistle(53 times),cortea eucommiae charcoal(52 times)and so on.3 In the statistical analysis of four natures,the frequency from high to low was cold,warm,mild,cool and heat.4 In the statistical analysis of five tastes,the times from high to low was bitter,sweet,spicy,sour,salty,astringent and light.And the frequency from high to low was bitter,sweet,spicy,sour,astringent,light and salty.5 In the statistical analysis of channel tropism,the frequency from high to low was liver meridian,kidney meridian,spleen meridian,stomach meridian and lung meridian.6 In the statistical analysis of efficacy,the frequency from high to low was tonic medicine,clearing heat medicine,hemostatic medicine,promoting blood circulation for removing blood stasis medicine,dispelling pathogenic wind and dampness medicine,astringent medicine,eliminating dampness and diuresis medicine and resolving exterior medicine.7 We could get six new Chinese medicine combinations by importing the core Chinese medicine in the prescription into SPSS24.0 software for cluster analysis: the first combination was radix cynanchi paniculati,twig of winged euonymus,caulis sinomenii,rhizoma smilacis glabrae,radix achyranthis bidentatae,rhizoma Chuanxiong and radix rehmanniae;the second combination was fructus corni and caulis piperis kadsurae;the third combination was folium pyrrosiae,radix paeoniae alba and rhizoma cimicifugae;the fourth combination was cortea eucommiae charcoal and field thistle;the fifth combination was semen euryales,fructus rosae laevigatae,cortex moutan and radix rubiae;the sixth combination was rhizoma imperatae,radix boehmeriae,herba ecliptae and fructus ligustri lucidi.Conclusion:According to the theory of kidney deficiency and blood stasis,Director Si Fuquan thought the pathogenesis of chronic nephritis hematuria could be summarized as deficiency,heat,dampness and blood stasis.The disease is located in kidney,involving lung,spleen,stomach and liver.On the basis of invigorating the kidney,promoting blood circulation and hemostasis,combined with the tongue and pulse and clinical symptoms,the treatment mainly includes clearing away heat and dampness,promoting blood circulation and removing blood stasis,eliminating wind and dampness,astringent hemostasis,and promoting water and dampness.The medicine mainly includes tonic,clearing heat,hemostatic,activating blood and removing blood stasis,eliminating wind and dampness,astringent,water and dampness,and surface resolving.It could be used peacefully,cold and warm,tonic and diarrhea simultaneously.
