| Objective: Objective: to explore the clinical characteristics of plasma cell mastitis and evaluate the therapeutic effects of different methods in order to take appropriate comprehensive treatment according to different characteristics of plasma cell mastitis.Methods: in the first part,55 breasts of 40 patients with I-II degree inverted nipple were treated in our hospital from January 2013 to December 2013.the clinical effect,the incidence of plasma cell mastitis and patient satisfaction were observed.Part two: from January 2014 to June 2019,250 patients with PCM(unilateral 242 cases,bilateral 8 cases)were retrospectively analyzed.They were divided into comprehensive treatment group and direct operation group.The comprehensive treatment group was given comprehensive treatment including anti-infection,anti-tuberculosis,puncture and empyema,incision and drainage,and surgical resection of the focus.The patients in the direct operation group were given anti-infective treatment when they were admitted to hospital,and then the lesions were resected according to the scope of the lesions.In the patients with nipple invagination in the two groups,traction nipple invagination correction and incision nipple invagination correction were performed to correct nipple invagination in degree I-II and degree III,respectively.Results: the first part: for the patients with I-II degree nipple invagination,the traction nipple invagination effect was significant(the effective rate was 94.5%),the incidence of plasma cell mastitis was 0%,and the patient satisfaction was good(95%).Part 2: there was no significant correlation between the degree of nipple invagination and the type of PCM.There was no significant difference in nipple invagination(),PCM type)and average hospital stay between the comprehensive treatment group and the direct operation group(p > 0.05).Both groups could achieve good therapeutic effect(effective rate 100%).The cure rate of the direct operation group was higher than that of the comprehensive treatment group(87.3% vs 73.4% score p < 0.05),and the recurrence rate was significantly lower than that of the comprehensive treatment group(6.0% vs 10.5% focus p < 0.05).However,the score of patient satisfaction in the comprehensive treatment group was significantly higher than that in the direct operation group(95.2% vs 78.4% Powerp < 0.05).Conclusion: 1.Traction correction of nipple invagination can achieve good results in the treatment of I-II degree nipple invagination.It has the advantages of simple operation,less injury and high aesthetics,so it is worth popularizing and can play a certain role in the prevention of plasma cell mastitis.2.The condition of plasma cell mastitis is complicated.In the comprehensive treatment group,anti-infection and anti-tuberculosis therapy combined with traction nipple correction can achieve the same effect as direct operation,and the cosmetic effect is significant,but the recurrence rate is higher than that in the operation group.3.For patients with PCM,a comprehensive treatment plan including nipple correction,anti-infection,anti-tuberculosis and operation should be given according to the presence or absence of nipple invagination,different clinical manifestations and patient wishes,which can achieve good treatment and cosmetic results. |
