Clinical Observation Of Yu-kun Decoction Combined With Climen In The Treatment Of Premature Ovarian Insufficiency Of Type Kidney-deficiency And Liver-stagnation

Objective:To observe the clinical efficacy of Yu-Kun Decoction combined with Climen in the treatment of premature ovarian insufficiency of type Kidney-Deficiency and Liver-Stagnation,and to subsequently provide new ideas for the treatment of the disease.Methods:The current study selected 84 patients with POI of type Kidney-Deficiency and Liver-Stagnation who were diagnosed in the gynaecological clinic of Shanxi Provincial Hospital of Traditional Chinese Medicine during January 2018 to October 2018.Those patients were randomly divided into three groups of 28,which were the group A,group B,and group C.Group A was treated with Yu-Kun Decoction combined with Climen,while group B was treated with Climen only and group C was treated with Yu-Kun decoction only.Data of a number of indicators before and after three treatment cycles were collected and analysed.Those factors are TCM syndromes,as well as Follicle Stimulating Hormone(FSH),Luteinizing Hormone(LH),Estradiol(E2),Anti-Mullerian Hormone(AMH),and Antral Follicle Count(AFC).Results:1.In the current study,firstly,patient’s age,duration of disease and various observation indicators(pre-treatment main symptom scores,secondary symptom scores,TCM syndrome total score;pre-treatment serum FSH、LH、E2、AMH;pre-treatment AFC)were compared between the three groups.The difference were insignificant(P>0.05),which suggested that these three groups were comparable.2.Disease efficacy index: After treatment,the total effective rate of group A was88.9%,the total effective rate of the group B was 69.2%,and the total effective rate of the group C was 57.1%.There was a statistically significant difference in the efficacy of the three groups(P<0.05);two-two comparison,there was a statistically significant difference between group A and group B as well as group A and group C(P<0.05),while there was no significant difference between group B and group C(P>0.05).3.TCM syndrome score index: The total scores of TCM syndromes in group A,group B and group C were lower than those before treatment,differences before and after treatment were statistically significant(P<0.05).After treatment,the difference between the three groups was statistically significant(P<0.05),two-two comparison,there was a statistically significant difference between group A and group B as well as group A and group C(P<0.05),while there was no significant difference between group B and group C(P>0.05).In improving the main symptoms of menstrual disorders,group A compared with group B,there was a statistically significant difference in the improvement of “menstrual period” and “color quality”(P<0.05),there was no significant difference in the improvement of “cycle” and “quantity”(P>0.05);group A compared with group C,there was a statistically significant difference in the improvement of “cycle” and “menstrual period”(P<0.05),there was no significant difference in the improvement of “quantity” and“color quality”(P>0.05).There was no significant difference in the improvement of group A and control group C(P>0.05),and they were excellent in improving the secondary symptoms of “soft waist and knees”,“hot and sweaty”,“breast pain” and “insomnia and dreams”,in group B(P<0.05).4.Serum sex hormone index: The levels of serum FSH and LH in group A,group B and group C decreased after treatment,and E2 increased compared with before treatment,differences before and after treatment were statistically significant(P<0.05).After treatment,the differences between the three groups were statistically significant(P<0.05),there was a statistically significant difference between group A and group B as well as group A and group C(P<0.05),while there was no significant difference between group B and group C(P>0.05).5.AMH indicator: AMH levels in group A,group B and group C increased slightly compared with before treatment,however,the difference before and after treatment was not statistically significant(P>0.05).There were no significant differences between the three groups after treatment(P>0.05).6.Ultrasound AFC index: The number of AFCs in group A,group B and group C improved compared with before treatment,differences before and after treatment were statistically significant(P<0.05).After treatment,the differences between the three groups were statistically significant(P<0.05),there was a statistically significant difference between group A and group B as well as group A and group C(P<0.05),while there was no significant difference between group B and group C(P>0.05).Conclusions:1)Yu-Kun Decoction combined with Climen can significantly improve TCM syndromes in patients with premature ovarian insufficiency of type Kidney-Deficiency and Liver-Stagnation,the curative effect is better than the single drug Climen and Yu-Kun Decoction.Yu-Kun Decoction has an advantage over the single drug Climen in improving the “soft waist and knees”、“hot and sweaty”、“breast pain”、“insomnia and more dreams”.2)Yu-Kun Decoction combined with Climen effectively reduced the levels of serum FSH and LH in patients with premature ovarian insufficiency of type Kidney-Deficiency and Liver-Stagnation,increased E2 level,and increased the number of AFC;the comprehensive curative effect was better than that of single drug Climen and Yu-Kun Decoction.
