Construction And Application Of Nursing Outcome's Sensitive Indexes Evaluation System In Perioperative Stage Of Lung Cancer Based On Nursing Outcomes Classification

Objective: To construct the index evaluation system of lung cancer perioperative period based on "classification of nursing outcomes ",and to apply this system to clinical nursing,to provide a reference for the future more scientific formulation of nursing plan for patients with lung cancer perioperative period,and to provide evaluation tools for evaluating nursing quality and nursing effect of patients with lung cancer perioperative period.Methods: The first part uses literature review method,group meeting method from the lung cancer perioperative core nursing diagnosis,corresponding to the fourth part of the book "classification of nursing outcomes " : NANDA international nursing diagnosis-link of NOC screening the corresponding outcome of nursing diagnosis,using the "field-end-indicator" three-level structure to formulate the questionnaire,through two rounds of Delphi expert letter method to determine the final inclusion of indicators,and then use AHP to set up the weight of each index,finally group meeting method and pre-practice adjustment of the questionnaire language habits,measurement,reliability and feasibility analysis.The second part uses the experimental study to carry on the system questionnaire empirical research,will accord with the Na pai standard 72 study subjects to carry on the random grouping,before the operation carries on the appraisal to the two groups outcome analysis,then carries on the nursing outcome intervention to the experimental group according to the individuality outcome.On the day of discharge,the outcome of the two groups was evaluated for the last time,and the improvement of the outcome of the two groups was observed,and the effect of the intervention on the outcome of the patients between 3 days after operation and discharge was analyzed.Results:1.architecture: the final formation of the "lung cancer perioperative outcome index evaluation system" contains 5 primary indicators,21 secondary indicators and 49 tertiary indicators.2.results of reliability analysis of the index of the letter of inquiry: the positivecoefficient of the two rounds of experts is 100%,the coefficient of authority of the two rounds is 0.837 and 0.860,respectively,and the degree of concentration and coordination of the two rounds of expert opinions better.The weight of the first level index is0.0757,0.2674,0.2978,0.0386,0.3205 respectively.The CR values of all levels <0.1.3.pre-experiment showed that the ? coefficient >0.7 and the KMO value was 0.7.4.empirical evaluation :1.Results of preoperative outcome evaluation showed that the two groups showed lower average of anxiety level,knowledge: diet and knowledge: health resources,lower average of family support during treatment,lower average of respiratory status in nurse evaluation: gas exchange,patient adaptation and lower average of family knowledge during treatment.3 days after operation,the results showed that the outcome of the patients in the two groups was in the position of change: active,self-care,tissue integrity:skin and mucous membrane,appetite,sleep before and after the operation scores were significantly different(P<0.05).outcome of the family and nurse assessment sections in the two groups showed significant differences in activity endurance,respiratory status: airway patency,tissue integrity: skin and mucosa,vital signs,respiratory status: scores before and after surgery for gas exchange(P<0.05).and for the intervention of the preoperative evaluation part,the score of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).Both groups scored higher than before,but the scores of tissue integrity were significantly higher than those of the control group(P<0.05).Conclusion: The evaluation system of perioperative outcome index of lung cancer has good reliability and clinical applicability,and can be used as a tool to evaluate the effect of nursing measures in patients with lung cancer perioperative period.Although routine nursing intervention can also improve the scores of patients' various outcomes,compared with the experimental group,the scores of the control group in some outcomes were significantly lower than those in the experimental group(P<0.05),which fully shows that nursing outcome intervention based on the evaluation system of perioperative outcome indicators of lung cancer has more advantages than routine nursing intervention.
