| Objective(s):Based on Omaha problem classification system and nursing outcome classification system,a scientific,specialized and applicable nursing sensitivity outcome index of localized tumor chemotherapy patients was constructed.To provide a tool to measure the effectiveness of nursing measures and nursing quality evaluation for cancer chemotherapy patients,so as to improve the quality of nursing and improve patient nursing outcomes.Methods:First of all,according to the results of literature review and clinical investigation,the research team initially formed a pool of nursing sensitivity outcome indicators for tumor chemotherapy patients on the theoretical basis of Omaha system and nursing outcome classification system;secondly,the Delphi expert letter inquiry method was used to implement three rounds of expert correspondence inquiries,combined with expert opinions to modify and improve the nursing perceptual outcome index of tumor chemotherapy patients.Finally,the analytic hierarchy process(AHP)was used to determine the weight of nursing sensitivity outcome index of tumor patients undergoing chemotherapy.Results:The nursing sensitivity outcome index of tumor chemotherapy patients were constructed by Delphi expert letter inquiry method,including 4 first-level indexes of physiology,psychosocial,health-related behavior and environment,13 second-level indexes of gastrointestinal toxicity,hematological toxicity and urinary toxicity,and 43third-level indexes of fluid balance change,frequency/intensity of nausea and vomiting,nature/intensity of pain.The results of three rounds of expert letter inquiries are as follows:the recovery rate of the questionnaire was 100%,90.9%,100%,respectively,and the experts are highly active to this study;The expert authority coefficient Cr values were0.913,0.955,0.955 respectively,and the Cr values are all more than 0.7,indicating that the experts have a high degree of authority and the result of letter inquiry is reliable.The coordination coefficients(Kendall’s W)of the three rounds of expert letters were0.250-0.227,0.251-0.261,0.205-0.225,respectively,the data were all more than 0.2 and the results were statistically significant(P < 0.05).The CV values of the three rounds of expert letters were 0.000-0.493,0.000-0.198 and 0.000-0.154 respectively.All these data indicate that the experts had a good degree of expert coordination.On the basis of the expert’s evaluation of the importance of the index,the analytic hierarchy process is used to calculate the weight of the index and test the consistency of the index.Among the three indexes,the weights of the first were physiological 0.424,psychosocial 0.227,health-related behavior 0.227 and environmental 0.122.The higher weights in the second indexes were self-care and caregiving supervision 0.151,anxiety0.123 and health 0.1223.Among the three indexes,the higher weights were 0.092 for adopting effective strategies to deal with anxiety,0.059 for understanding the importance of personal hygiene,and 0.092 for color/volume of urine.The CR values of the indexes at all levels are all less than 0.1,indicating that all the indexes in the index system have satisfactory consistency test results.Conclusion(s):In this study,the nursing sensitivity outcome indexes of tumor chemotherapy patients were constructed and calculated through status investigation,Delphi expert letter inquiry and analytic hierarchy process.The constructed indexes were scientific,reliable,authoritative,specialized and practical,and the weight distribution of indexes at all levels was reasonable.It can be used to quantify and evaluate the effect of patient nursing service,and provide reference for the nursing quality evaluation of tumor chemotherapy patients. |
