| Objective:In this study,the detection rates of Helicobacter pylori(H.pylori)and various periodontal pathogens in patients with severe periodontitis and non-severe periodontitis were conducted to analyze the correlation between H.pylori infection and severe periodontitis and the association between H.pylori infection with periodontal pathogens,it may reveal whether H.pylori infection is a risk factor for severe periodontitisMethods:Fifty-three patients who were treated in the department of periodontics of the School and Hospital of Stomatology to China Medical University from April 2018 to May 2019 were randomly selected.Based on the results of periodontal examination,the study subjects were divided into severe periodontitis group and non-severe periodontitis group.Subgingival plaque and serum samples were collected from all subjects.The H.pylori IgG antibody kits were used to detect H.pylori antibodies in the serum Subgingival plaque DNA was extracted and Polymerase Chain Reaction(PCR)was used to detect the presence of H.pylori,Porphyromonas gingivalis(P.gingivalis),Prevoella intermedia(P.intermedia),Fusobacterium nuclearum(F.nuclearum)and Treponema denticola(T.denticola)in subgingival plaque.Age and weight are expressed as mean ±standard deviation,T test is used for comparison between the two groups;gender and detection rate of each bacterium are expressed as frequency(percentage).Chi-square test is used for comparison between groups.Univariate analysis was performed using Odds Ratio(OR),chi-square test,and 95%confidence interval(CI).Variables considered to be statistically significant in univariate analysis were considered for logistic regression analysis.Statistical analysis was performed on the obtained data with SPSS 25.0Results:1.After adjusting for age factors by Logistic regression model,the risk of severe periodontitis in the subgingival plaque H.pylori infection group was 4.16 times that of the subgingival plaque H.pylori non-infection group(P<0.05);2.The risk of severe periodontitis in the current infection of H.pylori serum group was 5.04 times that of the serum H.pylori negative group(P<0.05);3.The risk of H.pylori in subgingival plaque was detected in the current infection of H.pylori serum group was 22.50 times that of the serum H.pylori negative group(P<0.05);4.The positive rate of F.nuclearum in the subgingival plaque in H.pylori positive group was 6.756 times higher than that in the subgingival plaque H.pylori negative group(P<0.05)Conclusions:This study suggests that H.pylori infection in subgingival plaque may be a risk factor for severe periodontitis.H.pylori current infection in serum may increase the risk of severe periodontitis.And H.pylori and F.nuclearum may exist the phenomenon of co-aggregation in the subgingival plaque environment.However,to further clarify the correlation between H.pylori and periodontitis,more large-scale,high-quality clinical studies are needed. |
