Research On The Constructing Model Of Post Competency On Attending Physician Of General Practitioner Of China

Objective: Based on the research of the general practitioner's post competency,this paper initially constructs the general practitioner's post competency model,aiming to study whether it is necessary to construct the post competency model according to different levels of the general practitioner,and at the same time,to provide the basis for standardizing the training of the general practitioner,selecting and evaluating the general practitioner in medical institutions and professional title evaluation.Methods:(1)using the literature research method to collect and sort out the research literature about general practitioners and post competency at home and abroad,and summarize the post competency items.(2)Use behavior event interview to further supplement and improve the post competency items.(3)The first round of Delphi method is used to screen,modify and perfect the general practitioner's post competency index;the second round of Delphi method is based on the general practitioner's post competency index system to initially build the general practitioner's post competency model,and calculate the authority coefficient,enthusiasm degree,opinion concentration degree,variation coefficient and coordination coefficient of experts.(4)The index weight is calculated by AHP.Excel and spss22.0 are used for data entry and statistical processing.Results:(1)The domestic literatures on general practitioners and post competency were summarized,and the results were used as the source of post competency index pool.(2)On this basis,the use of behavioral event interviews to supplement and improve the items,forming a competency model framework of level one indicators(6)and level two indicators(80).(3)In the first round of Delphi expert consultation,the recovery rate of the questionnaire is as high as 98%,adopting expert opinions and combining with group discussion,the first level indicators are retained and modified in words;80 second level indicators are adjusted to 63 second level indicators,and the connotation of the indicators is modified and improved.The concordance coefficient was 0.246,which was statistically significant(p<0.05).(4)Through the second round of Delphi expert consultation,the competency model of general practitioners was initially constructed,and the recovery rate of the questionnaire reached 87%,indicating that experts have a high enthusiasm for this study;the authority coefficient of experts reached 0.9073,indicating that the authority and reliability of experts are very high;the first level indicators are basic medical and health services,basic public health services,and information utilization Ability and management ability,medical knowledge and lifelong learning,interpersonal communication and team work,professional quality.The first level index coordination coefficient was 0.351,the second level index coordination coefficient was 0.231,which was statistically significant(p <0.05).Conclusion: The competency model of general practitioners in China consists of six first-class indicators and sixty-three second-class indicators,the first level indicators include A.basic medical and health services,B.basic public health services,C.information utilization and management ability,D.medical knowledge and lifelong learning,e.interpersonal communication and team cooperation,F.professional quality;the second level indicators are class A 13,class B 8,and class Crespectively Class C 10,class D 10,class E 10,class F 12.The evaluation index of the general practitioner's post competency preliminarily constructed in this study is highly effective and scientific,which can provide reference for the training and evaluation of the general practitioner in the future,and also has important significance for standardizing the assessment of the general practitioner and the examination of professional titles.
