| Objective:Preparation of elderly chronic disease in yunnan province family caregivers a supportive service demand questionnaire,and carries on the test reliability and validity of,and use the questionnaire of elderly chronic family caregivers in yunnan province,the questionnaire survey to investigate senile chronic disease in yunnan province family caregivers support services demand present situation and the main influence factors,for the communities,professional medical institutions and policy department to cany out targeted caregivers provide evidence supporting services.Methods:1.Semi-structured qualitative interviews were conducted with 13 family caregivers from yunnan geriatric hospital to analyze the content of supportive service needs of family caregivers with chronic diseases of the elderly.2.on the basis of a lot of reading literature,to Lazarus”stress and coping mode " as the theoretical guidance,establishment of elderly chronic disease in yunnan province and connecting with the actual interview results family caregivers supporting service demand questionnaire,experts consultation,preliminary investigation,and the two medical machine,the questionnaire survey to 220 family caregivers to test reliability and validity of it.3.A questionnaire survey of 320 family caregivers in 2 medical institutions in yunnan province was conducted by using the supportive service demand questionnaire of elderly chronic family caregivers in yunnan province to analyze the needs of caregivers and their influencing factors.Results:1.senile chronic disease in yunnan province family caregivers supportive service demand questionnaire exploratory factor analysis was used to extract the four common factor,the cumulative variance contribution rate is 77.01%,4 of each factor were named as "individual emotional needs","community support services demand","professional medical institutions supporting service requirements","policy needs" a total of 25 items;The overall retest reliability coefficient of the questionnaire was 0.801 and Cronbach’s alpha coefficient was 0.945.The content validity index(i-cvi)of all items in the questionnaire was more than 0.8,the content validity index(s-cvi)of the overall level of the questionnaire was 0.92,and the correlation coefficient between all items and the total score of the questionnaire was 0.378-0.837.2.40.92%of the elderly patients with chronic diseases had been sick for 20 to 29 years,the number of patients with chronic diseases of different ages was different(χ2=13.36,p<0.05),and 54.8%of the patients had impaired ADL function.Among the caregivers,women were significantly more than men(χ2=65.529,p<0.001).The mean age of the caregivers(60.4±10.6 years old)and 75.9%of the caregivers showed general health status.3.family caregivers in the process of taking care of elderly patients have higher requirements(93.82+/-16.13),mainly for the professional medical support services demand(42.66+/-5.52),followed by community support services demand(23.53+/-7.61),personal feelings should be(14.19+/-4.52)and policy requirements(13.4+/-2.99),the entry of the highest demand of 3 items as follows:in the elderly medication related medical information,community policy and professional telephone for help;The three lowest needs:the community provides care,food delivery and companionship services for the elderly.4.The total score of supportive service demand of female caregivers is higher than that of male caregivers in the elderly family caregivers with chronic diseases(t=-5.50,p<0.05),The needs of caregivers of different ages are different(F=5.409,p<0.05),among which the total score of needs of caregivers aged 40-59 is the highest.5.The results of multiple linear regression analysis showed that ADL,relationship with caregivers,health status of caregivers,care time and age of caregivers in elderly patients with chronic diseases were the main factors influencing the demand for supportive services of elderly family caregivers.Conclusions:1.The questionnaire on the supportive service needs of elderly family caregivers with chronic diseases has good reliability and validity,which can be used as an evaluation tool for the supportive service needs of elderly family caregivers with chronic diseases.2.Caregivers of different ages and genders have different needs for supportive services,and the factors influencing the care needs of this group are diversified.Relevant departments should provide targeted service intervention to caregivers to reduce their burden.3.Family caregivers play an important role in the context of the national old-age care service system that advocates "home-based old-age care and the combination of medical care and nursing care".With the increase in the prevalence of the elderly,to give corresponding support caregivers,give full play to the function of caregivers,not only can solve the problem of elderly patients with chronic disease care,also can make up for the community development is slow,patients needs and community service state of imbalance between supply and demand,but also conforms to our country the traditional concept of pension,is conducive to the family and social harmony.(l)implementing the "hospital-community-family" integrated management mode for elderly patients with chronic diseases and their family caregivers,establishing relevant laws and regulations for caregivers,and providing targeted interventions for caregivers can effectively reduce the burden level of caregivers and improve their care quality.(2)it is of great significance for the management and control of chronic diseases in China to broaden the role positioning of nurses,accelerate the training of general nurses,cultivate high-level community nurses and implement the right of prescription for nurses. |
