Objective:To investigate the preparation of the primary caregivers for home care patients when the patients are about to leave the hospital,and analyze the influencing factors;to explore the supportive service needs of the primary caregivers who are about to take the responsibility of home care patients,and to explore and propose the intervention strategies to promote the primary caregivers of the patients with first stroke to better prepare for home care,so as to help the patients and Family members make personalized continuous nursing service measures to provide reference,and ultimately achieve the purpose of promoting the recovery of patients and reducing the burden of caregivers.Methods:In this study,cross-sectional survey and qualitative interviews were used.The target population is the first stroke patients and their main caregivers who will be discharged from the Department of Neurology in a top three hospital in Taiyuan.The convenience sampling method was used to select 231 first-episode stroke patients and their main caregivers who were ready to leave hospital from March to October 2019.In addition,the purpose sampling method was used to conduct a semi-structured interview with the caregivers with the post discharge care needs as the research content.Finally,12 main caregivers were interviewed to achieve data saturation.The measurement data were analyzed by spss21.0 software,including descriptive analysis,t-test,variance analysis and multiple linear regression analysis.Inspection level ? = 0.05.The counting datawere analyzed by using the phenomenological analysis method of Colaizzi with the help of nvivo11 software.Results:1.The total score of the primary caregivers preparing for home care was(52.58 ±11.689).Among them,the scores of caregiver role related knowledge,disease process knowledge,economic source knowledge,social support for care,and confidence in home care ability were lower.2.Single factor analysis In terms of caregivers: different ages,occupations,current working status,education level,relationship between family and patients,chronic disease,per capita monthly income of court,living with patients,other caregivers,care experience,pressure of consciously caring for patients,conscious health status,patients:education level,occupation,payment method of medical expenses,self The level of management ability and whether there is a convenient hospital in the place of residence have significant influence on the preparation of the main caregivers for home care of the first stroke patients(P < 0.05)3.The results of multiple linear regression analysis showed that the age,occupation,monthly income per capita of the caregiver,whether there is care experience,whether there are other caregivers,the degree of pressure to consciously care for the patient,the level of self-care ability of the patient,whether there is a hospital in the place of residence that is convenient to see a doctor entered the regression equation of the caregiver's readiness for family care(main influencing factors(F = 40.359,P < 0.0)01),which can explain 57.8% of the variation of the caregiver's readiness for home care.4.Through the analysis of interview data,the main caregivers' needs for supportive services when the first stroke patients are about to leave hospital can be summarized into four themes: the desire for professional support,the desire for economic support,the desire for family and social support.Conclusion:1.The main caregivers' preparation for home care of the first stroke patients is at a medium level.The caregivers' age,occupation,monthly income per capita of the family,whether they have care experience,whether they have other caregivers,the degree of pressure to consciously care for the patients,the level of self-care ability of the patients,and whether there is a hospital in the place of residence that is convenient for medical treatment affect the care readiness of the main caregivers The main factors.2.The study suggests that health departments and relevant institutions should improve the hospital community family three-way linkage continuous care mode to achieve continuous care for patients;medical staff should systematically evaluate the care readiness of caregivers,and identify the main influencing factors according to the evaluation results,In order to dynamically evaluate the effect of the intervention measures and provide continuous and targeted care services for the patients,so as to promote the recovery of the patients and reduce the burden of the caregivers.
