Objective:This study aims to understand the conditions and problems of primary subjects lecture training through the empirical research on the primary subjects lecture training of resident standardized training in Yunnan Province,On the basis of analyzing and summarizing the basic situation,policy awareness,training content,training form,assessment arrangement and other influencing factors,combined with the actual situation in Yunnan Province,to improve the quality of primary subjects lecture training,propose methods to improve primary subjects lecture training,and provide decision-making basis and reference for promoting and improving primary subjects lecture training.Methods:This study stratified national six resident standardized training bases in Yunnan Province,namely the First People's Hospital of Yunnan Province,the Second Affiliated Hospital of Kunming Medical University,Yan'an Hospital of Kunming City,the People's Hospital of Yuxi City,the First People's Hospital of Qujing City,and the People's Hospital of Xishuangbanna Prefecture as the investigation site,conducted on-site training materials such as primary subjects lecture training programs and training plans for the six bases.A stratified sampling method was adopted,and 541 trainees and 127 teachers were selected for questionnaire survey.37 intensive trainees,12 teachers,and 14 management personnel were selected for personal in-depth interviews or group interviews.The questionnaire survey included basic information,knowledge of the resident standardized training policy,primary subjects lecture training requirements,training content,training methods,assessment arrangements,training effects,management work and other aspects of evaluation and satisfaction.The interviews included understanding of the policies related to resident standardized training,the training outline of primary subjects lecture,the form of training,the necessity evaluation or demand situation of the construction of the question bank,and the views and suggestions on the content of primary subjects lecture training and the form of training.The collected questionnaire data and interview results were summarized,Epi-data3.1 was used to establish database entry questionnaire data,and SPSS21.0 statistical software package was used for statistical analysis such as chi-square test,rank sum test,multiple response,and binary logistic regression.Fisher's exact test was performed using Stata 12.0 statistical software.Results:1.The six resident standardized training bases surveyed carry out primary subjects lecture training according to the relevant documents,but the differences of teaching content and teaching arrangement is large,and the specific implementation process is not standardized.2.1n terms of the standardized training policy awareness for residents,39.29%of the trainees and 56.20%of the teachers are aware of the resident standardized training policy.16.17%of the trainees and 7.44%of the teachers do not understand the standardized training objectives of the residents.For the knowledge of primary subjects lecture training requirements,43.98%of the trainees and 48.76%of the teachers are aware of the relevant requirements for primary subjects lecture training.46.99%of the trainees and 42.15%of the teachers have clear requirements for primary subjects lecture training.In the evaluation of the necessity of primary subjects lecture training,69.17%of the trainees and 92.56%of the teachers consider it necessary to carry out primary subjects lecture training.3.Regarding the evaluation of the primary subjects lecture training module(medical humanities+medical general knowledge+policies and regulations),89.29%of the trainees agree with the current primary subjects lecture training module,and 93.39%of the teachers believe that the current training module composition is reasonable.Regarding the evaluation and selection of primary subjects lecture training forms,29.70%of the trainees and 12.40%of the teachers believe that the current form of primary subjects lecture training "autonomous learning+distance learning"is not reasonable;In the choice of primary subjects lecture training form,52.26%of the trainees chose "self-learning+distance learning+base centralized teaching",and the selection ratio is the highest;In the primary subjects lecture of various training forms the choice of the proportion of hours(distance teaching:independent learning:centralized teaching),29.51%of the trainees chose "2:1:1",the selection rate is the highest.4.In terms of self-learning,15.60%of the trainees believe that the importance of independent learning in primary subjects lecture is generally or not important.59.97%of the trainees think that their own learning in primary subjects lecture training is generally not good or bad;In distance learning:42.48%of the trainees conducting online learning everyday,33.65%of the trainees have a brushing behavior,7.89%of the trainees have a substitute behavior;44.36%of the trainees and 84.30%of theteachers believe that the current distance learning of 100 hours are reasonable;38.16%of the trainees are dissatisfied with the satisfaction of the distance courseware content,and 32.32%of the trainees are generally or dissatisfied with the teaching quality of the current primary subjects lecture distance teaching.In the choice of the method of making distance learning courseware for primary subjects lecture,51.88%of the trainees choose the lecturer to appear on the screen,which accouts for the highest proportion;64.10%of the trainees think that the effect of distance learning is neither good nor bad;In the base centralied teaching:57.71%of the trainees and 82.64%of the teachers consider it necessary to carry out centralized teaching at the base;36.11%of the teachers choose "lectures"for the centralized teaching of primary subjects lecture in the base,which has the highest proportion;In terms of the time selection for the centralized teaching of the base,42.15%of teachers choose the time after work,which can be decentralized from Monday to Friday,the proportion is the highest;87.60%of teachers are willing to take on the relevant task of centralized teaching.5.In terms of primary subjects lecture assessment,92.86%of the trainees pass the primary subjects lecture assessment at one time;58.83%of the trainees consider that the amount of assessment questions is reasonable,and the trainees of 2016 grade(60.42%)consider the reasonable proportion to be higher than the trainees of 2017 grade(57.53%);In the assessment of the difficulty of the examination questions,58.27%of the trainees feel that the examination questions are difficult,39.10%of the trainees think it is very difficult or difficult;49.63%of the trainees are satisfied with the current primary subjects lecture assessment schedule(in the middle and late December of each year),79.34%of the teachers believe that the current assessment time is reasonable;In the specific time schedule,54.51%of the trainees choose to "set the assessment time zone,and the participants select the time exam during the period",which has the highest proportion;In the evaluation of the satisfaction of the assessment site,52.26%of the trainees are generally or dissatisfied with the current assessment location,and the trainees of 2016 grade(45.83%)is lower than the trainees of 2017 grade(49.31%);In the choice of the next residential primary subjects lecture assessment site,54.14%of the trainees chose to arrange the test sites in the residential base,which accounted for the most,followed by the city as the test area,accounting for 29.32%;74.43%of the trainees are satisfied with the current method of computer assessment.In the construction of the primary subjects lecture test bank,95.04%of the teachers consider it necessary to set up the test question bank.36.47%of the trainees chose to use the same set of test papers uniformly,with the highest selection rate,followed by 33.65%of the trainees choosing to build the test question bank,on-site random group questions."6.In the evaluation of primary subjects lecture training management,40.98%of the trainees rate the base teaching management satisfaction as general or dissatisfied,and 39.29%of the trainees rate the primary subjects lecture training distance teaching management as general or bad;The teachers believe that the trainees have insufficient attention(19.59%),low enthusiasm for learning(19.59%),and formalism(17.28%)in primary subjects lecture training.They believe that the base is not active in primary subjects lecture training(21.41%),training flows in the form(16.82%)and teachers do not pay enough attention(16.82%).In the formulation of the primary subjects lecture training syllabus,68.42%of the trainees and 88.43%of the teachers considered it necessary to formulate a primary subjects lecture training syllabus;In terms of teacher's choice of what should be included in the training syllabus,and the highest proportion is the specific training content(14.99%),training content module(14.72%),training form(14.03%),etc.7.In terms of the influencing factors of the effectiveness of primary subjects lecture training,28.12%of the trainees think that they are too busy,not too much time for study,and the proportion is the highest.Secondly,there is no unified training program,accounting for 20.99%.Lack of training material(20.75%)is also a factor that most trainees think affects learning effectiveness.Regarding the evaluation of the effect of primary subjects lecture training,54.14%of the trainees believe that the training effect is not ideal.The higher the grade,the unit person,knowledge of the relevant requirements of primary subjects lecture training,the trainees who think that the distance teaching effect and the training form are good,think that the training effect is good.23.97%of the teachers believe that the primary subjects lecture training is not effective,understand the objectives of the resident standardized training and the primary subjects lecture training syllabus,and consider the current primary subjects lecture training form as good for teachers,think that the current primary subjects lecture training effect is good.8.The resident standardized training trainees,managers and teachers of the resident doctors all talked about their views on the content of the primary subjects lecture training,the form of training,and the specific arrangements for assessment.The interviewees generally report that the current training content covers a wide range,and can still meet the needs of the resident standardized training.But some knowledge points suggest further expansion and in-depth,and timely update courseware.On the basis of the current training form,the main proposal is to increase the concentration of centralized teaching,while standardizing the training of primary subjects lecture teaching teachers.Most interviewees are satisfied with the current time,place,and form of assessment,and propose to build a primary subjects lecture test bank and develop a training program to improve the training program.Conclusions:1.The primary subjects lecture training for resident standardization training need to be improved;2.The awareness of resident standardized training policy is not high;3.Knowledge rate of the primary subjects lecture training requirements for resident standardization training is relatively low;4.The content of primary subjects lecture training for resident standardization training is not comprehensive and specific;5.The primary subjects lecture training for resident standardization training form is single;6.The primary subjects lecture training assessment for resident standardization training need to be standardized;7.The primary subjects lecture training system for resident standardization training is not perfect.Suggestions:1.Improve the primary subjects lecture training program for resident standardization training;2.Increase the publicity of the standardized training policy for residents;3.Strengthen the study and interpretation of primary subjects lecture training for resident standardization training;4.Expand and deepen the content of primary subjects lecture training for resident standardization training;5.Enrich the form of primary subjects lecture training for resident standardization training;6.Primary subjects lecture assessment work for resident standardization training needs to be adjusted and improved;7.Establish and improve the primary subjects lecture training system for the resident standardization training.
