| Purpose:The standardized training of resident doctors(hereinafter referred to as "residential training")is the core measure to promote the high-quality development of general practitioners.However,the turnover rate of general practitioners after completion of residential training is high,and the problem of the development and stability of the talent team of general practitioners persists.This paper conducts a questionnaire survey on the graduates of residential training of general practitioners in Chengdu to understand their current working situation and employment differences,explore the influencing factors of engaging in general practice,and put forward suggestions and reference basis for stabilizing and expanding the team of general practitioners.Method:By studying a large number of literatures,concepts and policies related to this study,the self-designed "Work Status Questionnaire of Trainees completing Standardized Training of Resident General Practitioners in Chengdu" was adopted,and a total of 479 trainees of resident general practitioners in Sichuan Provincial People’s Hospital,Chengdu Third People’s Hospital and the First Affiliated Hospital of Chengdu Medical College were selected as research objects.The questionnaire link was sent to the group via wechat.The data are input and sorted through the Chi-square test,rank sum test and Logistical regression for statistical analysis.Results:1.A total of 479 questionnaires were sent out in this study,and 415 were effectively received,with effective recovery rate of 86.64%.2.Basic information of work: 55.66% and 44.34% of residential trainees work in urban areas and towns,respectively,with Chengdu accounting for the largest proportion(45.06%)in regional distribution;40.96%,34.69% and 16.63% worked in general hospitals,community health service centers(stations)and township hospitals,respectively.Most of the trainees were employed in establishment and junior professional titles(62.41% and 56.39%),87.95% of them did not hold positions;3.Working status: 76.14% of the students registered for general practice after completing residential training,but only 38.80% of the students engaged in general practice.Among the students who did not engage in general practice,most chose to engage in internal medicine(51.18%).The average daily working time of 73.49% students was more than 8hours;77.35% of the students worked overtime > 1 time/month;53.01% of the students have less than 8 days off per month;The highest number(44.34%)are students with an annual salary of 40,000 to 80,000 yuan.The trainees are basically satisfied with the current working environment,the leadership’s attention,the policy of further study,the space to play the professional ability and the promotion and reward mechanism.59.52%of the students have the intention to quit.4.There were statistically significant differences in the work place,unit attribute,salary level,workload,dimission intention and post-dimission choice between students engaged in general practice and students engaged in non-general practice(P< 0.05);There were statistically significant differences between orientated medical students and nonorientated medical students in work place,unit attribute,employment method,salary level,workload,job satisfaction,dimission intention and post-dimission choice(P< 0.05).5.Female,postgraduate degree,rural household registration,The unit commissions trainees,and guidance from tutors during residential training are independent protective factors for the trainees to work in general practice after completion of residential training(P< 0.05).Conclusion:After completion of the program,the students of residential training need to improve their initiative to join the grassroots level,and their current job satisfaction is at a medium level,with large workload,less vacation,low salary and high turnover intention.The turnover rate of resident training doctors in general practice is higher after completion;Female,postgraduate degree,rural household registration,students commissioned by the unit,the guidance of the tutor during the residential training are the protection factors for general practice work;Although there are still some problems in the employment of resident doctors in general practice,the country has actively introduced policies to develop general practice.With the further implementation of these policies,the number of general practitioners will be stable and grow. |
