Objectives To understand the status and explore the influencing factors of infantile diarrhea care-seeking behavior intention,To provide an important scientific basis for the infantile diarrhea care-seeking behavior intention intervention strategies. Methods Based on the framework of the theory of planned behavior(TPB),the TPB was expanded through add the past care-seeking behavior, the “Infantile diarrhea care-seeking behavior intention scale” was developed, abbreviation “the scale”.At the same time, the “Scale” was tested for reliability and validity.Three township streets(Bali Town,Xiguoyuan Town and Jianlan Street)in Qilihe District of Lanzhou City were extracted by random cluster sampling method. From September 2016 to October 2016,a questionnaire survey was conducted among 649 children aged 0-36 months in this area. The questionnaire includes socio-demographic characteristics(basic situation of infants, basic situation of primary caregivers)and five modules(care-seeking attitude, subjective norm, perceived behavior control,care-seeking behavior intentions, and past care-seeking behavior)included in the questionnaire.Descriptive epidemiological methods were used to analysis personal characteristics and TPB dimensions.Path analysis was used to explore the direct or indirect effects of personal demographic characteristics' effect on the care-seeking behavior intentions.Bayesian structural equation model(BSEM)was used to explore the relationship between the dimensions of TPB and the influence of each dimension on the care-seeking behavior intentions, and compare with the traditional structural equation model(TSEM)to find out the main influencing factors of the infantile diarrhea care-seeking behavior intention. Results This study collected 620 valid questionnaires from three townships(streets)in Qilihe District of Lanzhou City.Path analysis showed that all path coefficients were statistically significant(P<0.05).The fitting index AGFI=0.979,GFI=0.991,NFI=0.976,RFI=0.955,and CFI=0.994 were all greater than 0.9.The total effect of the caregiver household registration type, the level of education of the caregiver, the relationship between the caregiver and the child, and the average monthly income of the family on the infantile diarrhea care-seeking behavior intention were 0.080,0.062,0.058,and 0.048,respectively.Results of BSEM indicated the model can explain 55.7% of the variation of the infantile diarrhea care-seeking behavior intention, which is higher than the results of other studies. The optimal BSEM was better than TSEM(BSEM: DIC=381,TSEM: DIC=719).The BSEM results showed that perceptual behavior control(?=0.731,P<0.05)and past care-seeking behavior(?=0.270,P<0.05)improved the infantile diarrhea care-seeking behavior intention,Subjective norms(?=-0.097,P<0.05)weakened their intention to care-seeking,care-seeking attitude had no significant effect on the care-seeking behavior intention.Perceptual behavior control had the greatest influence on the care-seeking behavior intention, and indirect effects on the care-seeking behavior intention through past care-seeking behavior. In addition,care-seeking attitude, subjective norms and perceptual behavior control interact with each other. The correlation coefficient between care-seeking attitude and perceptual behavior control was the largest(?=0.614,P<0.05),followed by care-seeking attitude and subjective norm(?=0.405,P<0.05).The correlation coefficient between subjective norm and perceptual behavior control was(? = 0.374,P<0.05)smallest.Multi-group structural equation model results indicate that model has no difference in model type and model parameters between different household registration types, different types of caregivers, average monthly income of different families, and different educational levels. The model has good stability and cross-group validity. Conclusions 1)The type of care recipient's household registration, the level of education of the caregiver, the relationship between the caregiver and the child, and the average monthly income of the family have an effect on the care-seeking behavior intention.2)BSEM has a better fitting effect on infantile diarrhea care-seeking behavior intention than TSEM.3)Subjective norms, perceptual behavior control and past care-seeking behavior have significantly effect on the infantile diarrhea care-seeking behavior intention. Therefore, the development of intervention strategies for infantile diarrhea care-seeking behavior intention should considerate of the influencing factors and taking into account the differences in socio-demographic characteristics.
