Objective:In addition to madopar-based conventional treatment,we comparatively observed the effects of extremities linkage training alone and extremities linkage training plus acupuncture combined with medicine(modified rehmanniae decoction of six ingredients plus acupuncture)on the motor function,balance capacity and activity of daily living of patients with Parkinson’s disease(PD)of liver and kidney deficiency.Besides,we preliminarily explored the clinical effectiveness of four-week extremities linkage training alone and modified rehmanniae decoction of six ingredients plus acupuncture.The aim of this study was to provide a reliable adjuvant therapy for the clinical treatment of PD.Methods:A randomized controlled trial(RCT)was conducted.66 patients with PD of liver and kidney deficiency who met the inclusion criteria were collected and randomly divided into two groups: experimental group(N=33)and control group(N=33).In addition to the use of madopar,the experimental group received acupuncture combined with medicine(modified rehmanniae decoction of six ingredients plus acupuncture)plus extremities linkage training for four weeks as a cycle.In addition to the use of madopar,the control group received extremities linkage training for four weeks as a cycle.The patients of the two groups were screened according to UPDRSⅡ,UPDRSIII,BBS and TCM tremor syndrome scale before and after treatment respectively,and the relevant results were recorded in detail.All the data were sorted out and statistically analyzed.By comparing the changes in the scores of UPDRS Ⅱ,UPDRS Ⅲ and BBS before and after treatment in the same group and the change rates of these scores after treatment between the two groups,the clinical effects of the two therapeutic regimens on the motor functions,balance capacities and daily activities of living of the two groups were evaluated.The changes of TCM syndrome in the two groups after treatment were compared by using TCM tremor syndrome scale,the overall response rates(ORR)of the two groups after treatment were calculated.Special attention was paid to modified rehmanniae decoction of six ingredients plus acupuncture on the TCM syndrome of patients with PD of liver and kidney insufficiency.Results:(1)A total of 66 subjects who met the inclusion criteria were enrolled.2 in the experimental group and 3 in the control group dropped out of the study,with a drop-out rate of 7.58%.A total of 61 subjects completed the whole treatment.During the process of treatment,the 6 patients had no obvious abnormal changes in vital signs such as blood pressure,pluses,respiration and body temperature,and blood routine,cardiac function,liver function,renal function and other laboratory indexes.(2)Intragroup comparisons of the scores of various scales of the experiment group and the control group before and after treatment: The scores of UPDRS II and UPDRS Ⅲof both groups after 4 weeks of treatment were obviously decreased than those before treatment,while those of BBS after 4 were remarkably increased than those before treatment,showing statistically significant differences(P<0.05).(3)Comparisons of the score change rates of various scales after treatment between the two groups: After 4 weeks of treatment,the score change rates of UPDRSⅡ,UPDRS Ⅲ and BBS scale of the experimental group were obviously higher than those of the control group,showing statistically significant differences(P<0.05).(4)The scores of TCM tremor syndrome scale of the experimental group and the control group after treatment were compared,and the ORRs of the two groups after treatment were calculated.The results showed that the score of the experimental group was obviously lower than that of the control group,showing a statistically significant difference(P<0.05);the ORR of the experimental group after treatment was remarkably higher than that of the control group,showing a statistically significant difference(P<0.05).Conclusions:(1)Both extremities linkage training and medicine combined with acupuncture(modified rehmanniae decoction of six ingredients plus acupuncture)are safe and reliable in treating patients with PD of liver and kidney deficiency,and they have good tolerance to the two therapies in clinical use.(2)Both extremities linkage training and herbs and acupuncture(modified rehmanniae decoction of six ingredients plus acupuncture)can effectively improve the motor function,balance capacity and activity of daily living of patients with PD of liver and kidney deficiency.(3)Comparison of the two therapies suggests that herbs and acupuncture(modified rehmanniae decoction of six ingredients plus acupuncture)outperform extremities linkage training alone in the overall improvement in the motor function,balance capacity and activity of daily living of patients with PD of liver and kidney deficiency.(4)Herbs and acupuncture(modified rehmanniae decoction of six ingredients plus acupuncture)can significantly improve the TCM symptoms and vital signs of patients with PD of liver and kidney insufficiency and enhance their quality of life. |
