Clinical Observation Of Xiaozhonghuoxue Decoction On Relieving Preoperative Swelling Of Schatzker Type Ⅳ, Ⅴ And Ⅵ Tibial Plateau Fractures

Objective:The patients of Schatzker type IV,V and VI of tibial plateau fractures,and TCM syndrome differentiation is qi stagnation and blood stasis syndrome,are treated with Xiaozhonghuoxue Decoction on the basis of conventional treatment,observe whether it can effectively relieve the swelling,reduce the preoperative preparation time,reduce the occurrence of deep vein thrombosis and tension of the blister.Methods:The subjects of this study were patients with tibial plateau fractures admitted to our hospital from January 2018 to February 2019,and 65 patients met the criteria of this study.According to the order of admission,the patients were randomly divided into the matched group(n=32)and the experimental group(n=33)by the random number table.After admission,both groups were given routine treatment,including calcaneus traction of the affected limb,ice compress,elevation of the affected limb,guiding the movement of the toe and ankle joint,drug detumescence,etc.Experimental group add Xiaozhonghuoxue Decoction on the basis of conventional treatment plus.Treatment courses in both groups began on the first day after admission and were stopped until the day of surgery.The preoperative preparation time,the incidence of deep vein thrombosis and tension blisters in the two groups were observed and recorded,as well as the swelling grade of soft tissue and swelling ratio of leg circumference before treatment,on the third day of treatment,on the fifth day of treatment and on the seventh day of treatment.All clinical data were statistically analyzed using the statistical software SPSS20.0.The changes of the two groups were analyzed and compared to evaluate the clinical efficacy of the two groups.Results:1.Before treatment,make comparison in age,gender,fracture site,fracture typing,swelling grading and swelling ratio of leg circumference,there were no statistically significant differences between the two groups(P > 0.05),which was comparable.2.Swelling grading: There were statistically significant differences in swelling grading between the two groups on the third day,the fifth day and the seventh day of treatment(P < 0.05).Intra-group comparison compared with that before the treatment: On the third day of treatment,there was a statistically significant difference in the matched group(P < 0.05),and the grade of swelling developed to a higher level.There was no significant difference in the experimental group(P>0.05).On the fifth day of treatment,there was no significant difference in the matched group(P>0.05).There was a statistically significant difference in the experimental group(P<0.05),and the grade of swelling developed to a lower level.On the 7th day of treatment,there were statistically significant differences between the two groups before and after treatment(P<0.05),and the grade of swelling in both groups developed to a lower level.3.Swelling ratio of leg circumference: There were statistically significant differences between the two groups in the swelling ratio of leg circumference on the third day,the fifth day and the seventh day of treatment(P<0.05).Intra-group comparison compared with that before treatment: On the third day of treatment,there were statistically significant differences between the two groups before and after treatment(P<0.05),and the swelling ratio of leg circumference in the matched group increased.The swelling ratio of leg circumference was decreased in the experimental group.On the fifth day and the seventh day of treatment,there were statistically significant differences between the two groups before and after treatment(P < 0.05),and the swelling ratio of leg circumference in both groups decreased.4.Preoperative preparation time: There were statistically significant differences between the two groups(P < 0.05).5.Positive rate of deep vein thrombosis: There were no statistically significant differences between the two groups(P > 0.05).6.Incidence of tension blisters: There were no statistically significant differences between the two groups(P > 0.05).Conclusion:For patients with Schatzker type IV,V and VI tibial plateau fractures,and TCM syndrome differentiation is qi stagnation and blood stasis syndrome,the addition of Xiaozhonghuoxue Decoction on the basis of conventional treatment can reduce swelling faster and better,and reduce preoperative preparation time,which deserve clinical promotion.
