Objective: Observation on the clinical efficacy of external application of Chinese herbal medicine(Ice-Nitrate Dispersion)in the treatment of perioperative swelling and pain of Schatzker type VI tibial plateau fracture.Methods: Sixty-two patients with Schatzker type VI tibial plateau fracture who met the inclusion criteria and voluntarily participated in the orthopaedic department of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine between September 2020 and December 2022 were selected and randomly divided into two groups:experimental group(31 patients treated with external application of ice nitrate combined with conventional swelling reduction)and control group(31 patients treated with conventional swelling reduction).In the control group,the patients were given heel traction treatment on the day of admission,ice packs wrapped around the towel around the knee joint(intermittent cold compresses for 40 minutes once,3 times a day for 48 hours),intravenous decongestant medication(sodium hepaticoside 40 mg + 0.9% sodium chloride injection 250ml),and elevation of the affected limb and In the experimental group,the patient was given ice-nitrate powder on top of the control group(once a day in the morning and once in the afternoon for 3 hours each time).The medical staff assessed the swelling and pain of the affected limb before admission,on the 3rd and 5th day after treatment,and observed the classification of the swelling of the affected limb,the swelling value of the affected limb,the rate of swelling reduction,the VAS score and the waiting time before surgery.The data obtained from the experiment were transferred to SPSS 26.0 statistical software and studied statistically in order to compare the differences between the two groups.Results:1.By analysing the results of this study,a statistical comparison of the two samples in terms of variables such as gender,distribution of affected sides,age,degree of swelling,swelling values and pre-treatment VAS scores yielded a difference of P > 0.05,which was not statistically significant.Compared with the two groups,the P value of the swelling grading on the third day of treatment and the fifth day of treatment was < 0.05,which was statistically different;compared with before treatment,the P value of the swelling grading on the third day of treatment was >0.05,which was not statistically different;the P value of the swelling grading on the fifth day of treatment was <0.05,which was statistically different from that before treatment.2.In the comparison between the two groups,there was a statistical difference between the two groups in terms of P-value <0.05 for swelling classification on day 3 and day 5 of treatment;in the comparison between the two groups,there was no statistical difference between the two groups in terms of P-value >0.05 for swelling classification on day 3 of treatment compared with that before treatment;there was a statistical difference between the two groups in terms of P-value <0.05 for swelling classification on day 5 of treatment compared with that before treatment.3.In the comparison between the two groups,the swelling values on the 3rd and 5th day of treatment were statistically different;in the comparison within the group,the difference was statistically significant when comparing the control group 3 days after treatment with the pre-treatment period.The difference was statistically significant;the difference was statistically significant when comparing 5 days after treatment with 3 days after treatment in the experimental group.4.The difference between the two groups was statistically significant,with P < 0.05 for a comparison of the swelling rate between the two groups at each stage of treatment and P < 0.05 for a comparison between the two groups at day 3 after treatment,and P <0.05 for a comparison between the two groups at day 5 after treatment.5.The difference between the two groups is statistically significant;the difference between the two groups is statistically significant;the difference between the two groups is statistically significant.The difference was statistically significant.The difference was statistically significant when comparing the experimental group with the pre-treatment group on day 3 after treatment,P < 0.05.The difference was statistically significant when comparing the experimental group with the pre-treatment group on day 5 after treatment.6.The difference was statistically significant at P < 0.05 for preoperative waiting time.Conclusion: The external application of Chinese herbal medicine(Bing Nian San)has good clinical efficacy in reducing the perioperative swelling and pain of Schatzker VI tibial plateau fractures and shortening the preoperative waiting time,which is worthy of application and promotion in the treatment of complex tibial plateau fractures with perioperative swelling and pain.
