Clinical Study On Xuanfeijiangzhuo Therapy In The Treatment Of AECOPD With Respiratory Failure

Objective:To observe the clinical efficacy of Xuanfeijiangzhuo therapy in the treatment of AECOPD with respiratory failure,and to monitor the changes of some laboratory indicators,evaluate the clinical efficacy of Xuanfeijiangzhuo therapy in the treatment of AECOPD with respiratory failure,and provide an effective treatment for AECOPD with respiratory failure.It is also used in clinical practice to improve the quality of life of patients and promote the application of TCM treatment and TCM theory in clinical practice.Methods:A total of 62 inpatients with AECOPD complicated with respiratory failure were enrolled in this trial.They were divided into treatment group and control group according to the method of random digital table,with 31 cases in each group.The control group was treated with Western medicine routinely.The treatment group was treated with traditional Chinese medicine decoction（Xuanbai Chengqi Decoction combined with Gualou Baibanxia Decoction）on the basis of conventional Western medicine treatment.The two groups were observed for 7 days.The symptoms,TCM syndrome scores,blood gas analysis and inflammatory indexes of the two groups before treatment,after 3 days of treatment,and after 7 days of treatment were compared.The safety of the whole observation course was monitored.SPSS20.0 was analyzed statistically to evaluate its clinical efficacy and safety.Results:（1）Baseline comparison:The baselines of the two groups were balanced and comparable.（2）Comparison of safety:No serious adverse reactions occurred during the observation in both groups,and the safety was well.（3）Clinical efficacy:Both groups could effectively improve the clinical symptoms and signs,but the treatment group control display rate was 73.3%,the effective rate was 93.3%,the control group control display rate was 46.6%,the effective rate was 83.3%,the treatment group better than the control group.（4）TCM symptom score:Both groups could improve symptoms and signs such as cough,expectoration,shortness of breath,wheezing and fever（P<0.05）,but the treatment group improved symptoms such as cough,expectoration,shortness of breath,wheezing and bloating better than the control group（P<0.05）.（5）TCM syndrome scores:Both groups could reduce TCM syndrome scores,but the treatment group was better than the control group（P<0.05）.（6）Blood gas analysis and oxygenation index:Both groups could improve PaO₂,PaO₂/FiO₂ in patients,and reduce PaCO₂ in patients,but the treatment group was better than the control group（P<0.05）.（7）SIRS index and inflammatory index:Both groups could improve patients T,P,WBC,N%,PCT,but the effect was equivalent.Conclusion:Xuanfeijiangzhuo therapy has a certain effect in the treatment of AECOPD combined with respiratory failure,can significantly improve the symptoms of cough,expectoration,shortness of breath,wheezing,fever,abdominal distension,etc.It can improve the arterial PaO₂,PaO₂/FiO₂,reduce PaCO₂,and no adverse events occurred.
