Objective:Based on literature analysis,semi-structured interviews and Delphi expert consultation,this study aims to construct an evaluation index system of graded care for the elderly in integrated medical and nursing institutions,clarify the evaluation focus of graded care for the elderly in integrated medical and nursing institutions,and provide a scientific and reliable reference basis for graded care evaluation for the elderly in integrated medical and nursing institutions.Methods:In this study,literature analysis,semi-structured interviews,delphi method and analytic hierarchy process were used to construct the evaluation index system of graded care for the elderly in medical and nursing institutions,and the weight of each index was determined.Data collation and analysis mainly use NVivo 7.0 qualitative data analysis software,Excel 2016,SPSS 23.0 and "Yaahp V6.0" AHP software.Results:1.Establishment of Graded Care Evaluation Index System for the Elderly in Medical and Nursing Integration InstitutionsAt present,the evaluation index system of graded care for the elderly in integrated medical and nursing institutions in China is not yet perfect.Based on the analysis of the current situation of graded care for the elderly in medical and nursing institutions in China,the experience of developed countries and the actual situation of China,through semi-structured interviews and two rounds of expert consultation,the evaluation index system of graded care for the elderly in medical and nursing institutions was established,including 4 first-level indicators(basic activity ability,physiological function,mental and psychological function,social function).It has 14 secondary indicators and 72 tertiary indicators.2.Selection of experts is representative and the results are scientific and reliable.The response rates of the two rounds of expert consultation questionnaires were 100%,88.46%,and the effective recovery rates were 100%.A total of 22 experts put forward constructive suggestions,reflecting the enthusiasm of experts for this study.The authoritative coefficients of experts in the two rounds of inquiries were 0.933 and 0.941,respectively,indicating that experts had a high degree of authority.The Kendall coordination coefficients of the two rounds of consultation expert opinions were 0.338 and 0.405,respectively,and all of them were P < 0.001,indicating that the experts' opinions on the indicators were relatively consistent.3.Determine the weights of each index in the evaluation index system of graded care for the elderly in medical and nursing institutionsThe weights of all levels of indicators are determined by analytic hierarchy process.The weights of the first level indicators from high to low are basic activity ability(0.4491),physiological function(0.3181),mental and psychological function(0.1553)and social function(0.0775).Conclusion:1.This study explored the concept and connotation of graded care for the elderly in medical and nursing institutions.Based on the comprehensive assessment of the elderly(CGA)and the International Classification of Function,Disability and Health(ICF),a graded care evaluation index system for the elderly in medical and nursing institutions was established by using semi-structured interviews,Delphi expert consultation and analytic hierarchy process,including 4 first-level indicators and 14 second-level indicators.The index and 72 three-level indicators have good weight coefficients at all levels.2.The content of this study is truthful,the method is scientific and reasonable,the positive coefficient and authoritative coefficient of consultants are high,the coordination of experts' opinions is good,and the research results are scientific and reliable.
