Objective:With the intensified of population aging and the demand for institutional care, the quality of life and safety for the older people in the pension agency are in more attention. Pension agency grade nursing system as the basis of care measures for the check-elderly and also as an important part of nursing management, which clearly defines the grading and nursing requirement of each level, plays an important role in the quality of life and conservation security to the elderly in the pension agency. The current national policy is lack of a uniform classification to the nursing grading system of pension agency, and most of pension agencies are choose to develop their own grading standards based on life care, which is general and vague, mostly taken different measures in different agency. Therefore, a significant difference between the nursing services of pension agency leads that the quality of care to the elderly can not be well protected. Orem theory is a mature used nursing theory, which aims to improve self-care ability of the individual, and its three nursing theory has good guidance for the development of the pension agency nursing grading standards. Based on this, this study is to establish a set of scientific, standardized nursing grading standard for pension agency, which is intends to give a objective and integrated assessment to the elderly to develop a scientific and a personal nursing program, and also can provides a reference to the government departments to introduce relevant policies.Methods : This research based on Orem theory, combined with qualitative and quantitative methods to build a nursing grading standard for the pension agency.1. Expert interviews were used to determine the first and second level indicators.2. A questionnaire survey was conducted in Chongqing in 211 elderly, which is to find out the chronic status, life satisfaction and self-care ability, and the questionnaire was designed based on literature analysis. Then a depth interviews was taken in 11 managers of pension agency of Chongqing to research specific situation pension agency nursing grading.3. Combined with theoretical study, literature analysis and In-depth interviews were used to develop three levels of indicators, and the nursing grading standard was established by summarized the opinions of 18 experts, through two rounds of expert consultation letter.5. A pension agency of Chongqing was taken as the basis, adopted 127 elderly people by convenience sampling method. The elderly were assessed by trained group members with the nursing standard which established by this research. The differences of each class constitute odds were assessed by two different methods, and tested by ?2 test, test level α = 0.05.Results: 1. Through in-depth interviews on the managers of pension agency,three themes were extracted: lack of a unified standard of nursing grading and a Professional evaluators team, the application of nursing grading were irregular, many difficulties in the management of nursing grading.2. A questionnaire survey was conducted for the institutionalized elderly, which found the prevalence rate of chronic diseases was 93.36 percent, while 80.09 percent of elderly suffered from two kinds of chronic diseases, professional care was large demanded, and the types of diseases, self-care ability, level of understanding of the diseasehe, and the frequency of health ducation from the nusing staffs were gave a significant effect on the satisfaction of institutionalized elderly.3. The nursing grading standard for pension agency was formed, which include five care levels(self-care, mediated assistance, nursing care, all care, and intensive care), 14 assessment contents, and 57 pension agency caring content. 2. Compared the differences of each class constitute odds which assessed by two different methods, the differences in self-care, mediated assistance, nursing care, full support level was statistically significant which tested by ?2 test, P <0.05.Conclusions:The results show that the nursing grading standard established by this study is scientific and reliable, can well matched to the health needs of the elderly, and it can prides a scientific basis for the nursing staff to take effective are for the elderly, but also can provide the basis for the deployment and examination of staff.
