Current Status Of Mild Cognitive Impairment In Community Elderly In Nanning City And Effect Evaluation Of Comprehensive Intervention Measures

Objective:(1)To understand the present situation and influencing factors of mild cognitive impairment in the elderly community in nanning,and to provide scientific basis for the establishment of the early intervention measures of MCI.(2)According to the risk factors of MCI,comprehensive intervention measures were developed to implement intervention in MCI patients in the community and evaluate the effectiveness of comprehensive intervention measures.To provide scientific basis of further carrying out early intervention in elderly patients with mild cognitive impairment in community.Methods:(1)The Beijing version of The Montreal cognitive assessment(MoCA-BJ)was used to evaluate The cognitive function of 3,000 elderly peopleaged 60 and above in 3 communities in nanning city by using The method of cluster random sampling.(2)One community in the baseline survey was selected by random sampling as the intervention implementation point,and 140 MCI who agreed to participate in the intervention study were divided into the intervention group and the control group according to the random number table method.The intervention group adopted a comprehensive intervention method combining health education,cognitive training and finger exercises.Results:(1)In this study,833 MCI patients were detected.The MCI standardized detection rate was 27.27% by the age composition of the population in Nanning in 2010.Single factor analysis showed that there were significant differences(all P<0.05)in the detection rate of MCI indifferent groups by age,education,occupation,exercise time,sleep time,number of reading,community activities and housework.Multivariate analysis showed that: old age,primary education level,occupation of farmers/migrant workers and enterprise personnel/workers are the risk factors of MCI,and Maintaining 6 hours or more of sleep,2 hours or less of exercise,and reading at least once a week were protective factors of MCI.(all P<0.05).(2)After the implementation of comprehensive intervention measures for 6 months,the total scores of MoCA-BJ cognitive scale,attention,language and recall in the intervention group were all higher than those in the control group,with statistically significant differences(P<0.05).Compared with the baseline,the MoCA-BJ cognitive scale score of the intervention group was improved on 6 months after intervention,and the difference was statisticallysignificant(P<0.05).Conclusions:(1)The detection rate of MCI is high among the elderly in Nanning.The elderly with advanced age,primary school education,occupation as farmer/peasant-worker and enterprise personnel/worker are at high risk of developing MCI.Effective measures should be taken as soon as possible to delay the occurrence and development of MCI.(2)Comprehensive intervention measures can effectively improve the overall cognitive function of elderly people in MCI community,especially the improvement on attention,language and memory.
