Objective:The purpose of this study is through literature rerview,cross-sectional survey,semi-structured interview and other methods to understand the status and influencing factors of post management of Chinese orthopaedic specialist nurses.Combined with the existing problems and post management theory,the post management system of Chinese orthopaedic nurses is constructed.It provides guidance for the planning of career development path for orthopedic nurses,provides reference for hospitals at all levels to do a good job in the use and retention of orthopedic nursing professionals,and provides reference for relevant national departments to improve the post management system for specialized nurses.Methods:(1)Literature review Domestic and foreign specialist nurses,orthopedic specialist nurses use and management systems and policies to determine the post management elements of orthopedic specialist nurses.(2)Adopting a convenient sampling method to extract 560 orthopedic specialist nurses from 28 provinces(municipalities directly under the central government)as the survey subjects,and investigate the current situation of postoperative management of orthopedic specialist nurses and the satisfaction of orthopedic specialist nurses through self-made questionnaires.(3)Adopting a semi-structured interview method,according to the interview outline,interviewed 15 orthopedic specialist nurses from 12 top three hospitals nationwide to clarify their work experience and needs.(4)Based on the relevant experience at home and abroad,combined with the current situation survey and qualitative interview results,draft a preliminary draft of the post management system for orthopedic specialist nurses.Through the Delphi expert consultation method,18 orthopedic clinical nursing,orthopedic nursing management and clinical medical experts from all regions of the country were invited to make two rounds of consultations to determine the post management system for orthopedic specialist nurses.Results:(1)Through questionnaire survey,it was found that 560 orthopedic specialist nurses from 28 provinces and 10 orthopedic nurse training bases had 56.94% satisfaction with job management,and about 10% had turnover intentions and job management satisfaction.At a medium level.74.64% of people think that they have no more development opportunities,"the treatment that does not reflect the value of the job","the lack of clear job responsibilities","the clinical work workload is too heavy","lack of government unified qualification certification",“ The lack of access to new knowledge technologies” is considered to be the top five issues.50.71% of the respondents did not have a specialist nurse management system,and 20% of them said that they were unclear,and the management systems between the hospitals were quite different.74.64% of the posts did not change after the training of orthopedic specialist nurses.The work content was still based on case care and nursing rounds.Only 10% of the people had experience in nursing clinics.The number of people who work within 1 set per week and overtime within 1 hour is 43.21%.301 people have not received relevant re-education training,but 72.86 people still have confidence in the development prospects of orthopedic specialist nurses.(2)Single factor analysis from the individual aspects of orthopedic specialist nurses,hospital management system and job work environment,and statistically significant single factor into binary logistic regression model,found that the postoperative satisfaction of orthopedic specialist nurses was late.The number of night shifts,psychological empowerment,professional identity,career planning management,fairness of payroll compensation,instrumentation,patient and family recognition,and leadership style of nursing managers.(3)Refining 8 themes for the work experience of orthopedic specialist nurses: improving the sense of professional benefit,highly valued by managers,vague positioning of their roles,unclear professional development direction,inconsistent positions,establishing qualification system,and setting up special nursing Team platform and build information communication platform.(4)Finally,five first-level indicators,17 second-level indicators and 95 third-level indicators,including “ post setting”,“employment management”,“performance appraisal”,“post training” and “post incentive”,were established.Oral specialist nurse position management system.Conclusion: At present,the work related to the use and management of specialist nurses has been launched in China.The current situation of post management is not optimistic,and the satisfaction of post management is at a medium level.It is extremely urgent to establish and improve the post management mechanism for orthopedic specialist nurses.The expert opinion of the orthopedic specialist nurse post management system formed by this institute has high consistency and feasibility,and can provide reference for the establishment of orthopedic specialist nurse post management system for all levels of departments.
