Correlative Research Among Pathology,TCM Syndrome And Physical Constitution Of Gastric Polyps

Objective:Under the guidance of TCM theory,through the clinical observation and study of gastric polyps in patients with pathologic type and TCM syndrome types,physical relevance,as to provide theoretical basis for preventing and controlling the disease with TCM,better promote the development and the combination of traditional Chinese medicine and modern medicine.Ways:The clinical data of 477 patients with gastric polyps were investigated and analyzed by epidemiological investigation.Observationincluded demographic chara cteristics,clinical symptoms,syndrome differentiation in patients with type,distingui shing constitution,gastroscopy and pathological histology,tongue and pulse conditio n and auxiliary examination,using SPSS 22.0 statistical analysis of data.Results:1.In patients with gastric polyps,163 male patients(34.2%)and 314 female patients(65.8%),The morbidity rate above 40 is high,accounting for 91.8%.2.The growth part of the gastric polyps was mostly in the stomach bottom and stomach body(78.9%),and the number of the gastric polyps was more than that of polyhedron(65.4%).The diameter of gastric polyp was less than 0.5cm(61.6%),and it is rarely greater than 1.0cm(4.2%).The surface morphology is mainly smooth(75.7%),mulberry and leaves at least(2.3%);Patients with Hp positive accounted for 20.5%.3.Gastric fundus polyps of patients(56.8%)were the most common pathological types,adenomatous polyps(2.1%)were the least.There was no correlation between the pathological types of polyps and their gender,age,number of polyps,and the size of the diameter of polyps(P>0.05).There were significant differences in the pathological type distribution of polyp growth site,surface morphology and infection of Hp(P<0.05).The gastric body was mostly seen with gastric fundus,and the gastric antrum was seen with proliferative polyps and inflammatory polyps.The surface morphology of adenomatous polyps mainly consists of mulberry and leaf.Inflammatory polyps and hyperplastic polypswere associated with Hp infection.4.In the patient’s syndrome type,spleen and stomach humidity and heat syndrome(29.6%)>liver and stomach syndrome(28.7%)>spleen and stomach weak syndrome(18.0%)> phlegm-dampness syndrome(17.4%)> gastric blood stasis syndrome(6.3%);There were significant differences between the patients’ gender and whether they were infected with Hp(P<0.05),and women with a majority of liver and stomach dissonance.The Hp positive rate is higher in spleen and stomach.In patients with different pathological types,there was a significant difference in the syndrome distribution(P<0.05),and the proliferative polyps were mostly in the spleen and stomach,and the inflammatory polyps were mostly in liver and stomach.5.In the patient’s physical type,constitution of qi stagnation(24.1%)> constitution of damp-heat(21.4%)>constitution of yang asthenia(16.6%)>constitution of qi asthenia(13.2%)> constitution of phlegm-dampness(12.4%)> constitution of yin asthenia(4.0%)> constitution of blood stasis(3.8%)= constitution of yin-yang harmony(3.8%)>allergic constitution(0.8%).In patients with different genders,there was a significant difference in their physical distribution(P<0.05),and women were mainly in constitution of qi stagnation,while men were mostly constitution of damp-heat.There was no significant difference in the physical type distribution of patients with different pathological types(P>0.05);Different constitution types of patients,the card type distribution have significant difference(P<0.05),Heat and humidity of spleen and stomach were mainly constitution of damp-heat is qualitative,The liver and stomach disharmony is dominated by constitution of qi stagnation,The deficiency of spleen and stomach is dominated by constitution of yang asthenia,constitution of qi asthenia;The constitution of qi stagnation are mostly in the liver and stomach,and the constitution of damp-heatis seen in the spleen and stomach.6.The number of patients with adenomatous polyp(10 cases)collected in this study was not included in the statistical study of syndrome type and constitution correlation.But clinical observation of TCM syndrome distribution: 2cases of spleen and stomach dampness,1 case of weakness of spleen and stomach,3 cases of obstruction in phlegm,4 cases of blood stasis in stomach.Its constitution type distribution: 2 cases of constitution of qi asthenia,2 cases of constitution of yang asthenia,2 cases of constitution of phlegm-dampness,1 case of constitution of damp-heat,3 cases of constitution of blood stasis.Conclusion: 1.The pathologic type of polyp in patients is related to the syndrome type: hyperplastic polyps are mainly in the spleen and stomach,and the inflammatory polyps are mostly in the liver and stomach,and the gastric fundus is seen in various syndromes;2.The pathologic type of the patient was not related to its constitution type;3.Patients with syndrome types and its constitution type correlation,Heat and humidity of spleen and stomach were mainly constitution of damp-heat is qualitative,The liver and stomach disharmony is dominated by constitution of qi stagnation,The deficiency of spleen and stomach is dominated by constitution of yang asthenia,constitution of qi asthenia;The constitution of qi stagnation are mostly in the liver and stomach,and the constitution of damp-heatis seen in the spleen and stomach.
