Objective:The aim of the study is to utilize the clinical features and pathological types of polyps under endoscopy of patients with gastric polyps from the First Affiliated Hospital of Dalian Medical University for the analysis of the clinical epidemiology of gastric polyps in recent years,the prevention of stomach polyps.And finally to make the diagnosis identify pathological types and select the appropriate method.Material and Methods1.SubjectsA total of 628 hospitalized patients in the Department of Gastroenterology from the First Affiliated Hospital of Dalian Medical University from January 2014 to December 2016 were enrolled.Except for gastric cancer,Peutz Jeghers syndrome(Peutz-Jeghers),juvenile polyposis syndrome(JPS)or combined with other conditions such as stomach surgery.Collecting the clinical data and education background,living condition,along with other systems diseases,auxiliary examinations and gastroscopy results.Endoscopy results of polyps must be carefully noted,including the number,the maximum diameter(choose the maximum diameter if the polyps were multiple),pedicel,mucosal surface.The group consisted of 146 males(23.3%)and 482 females(76.8%),the gender ratio was 1:3.13.The youngest patient was 21 years old,and the oldest one 86 years old.The average age was 59.24 years old.2.EndoscopyChecked the esophageal and every part of the stomach by the professional endoscopic doctor,then recorded the informations of polyps,including the location,size of polyp,protrusion or flat,and the mucosal surfaces,and so on.3.Histopathological ExaminationWe chose directly electric coagulation resection for a single polyp and for which was diameter no more than 3mm.Meanwhile,for the polyps that was bigger than 3mm,we chose to cut off and select small tissue for biopsy.For the multiple gastric polyps,we chose 1 or 2 tissues for biopsy.Pathological diagnosis of all samples were made by by the hospital pathology.4.Statistical ProcessingStatistical analysis was done useing SPSS 21.0.Data were expressed as x±S or median.Chisquare test was used to test differences between the two groups of patients.P values<0.05 were considered statistically significant.Results1.The age of 628 patients ranged from 21 to 86 years old,and the mean age was 59.24.Among them,compared to the female and male patients,the incidence of patients in different age groups was different(X2=9.585,P=0.048).Male patients accounted for 23.2%(146/628),and female patients accounted for 76.8%(482/628),.The ratio of male and female patients was 1:3.13.The incidence rate of female was more than male patients with significant difference(X2=628,P<0.001).2.There were a series of clinical symptoms,including acid reflux(18.1%),heart bum(15.9%),burning sensation of sternum(or pharyngeal swallow,dysphagia)(3.7%),weight loss,and anorexia(2%),nausea and vomiting(5.2%),abdominal pain(20.9%),abdominal distension(16.3%),abdominal discomfort(5.6%).3.Among the 628 patients with gastric polyps,the solitary polyps were 333 cases(53%),and multiple polyps were 295 cases(47%).The most common type was gland polyps(49%),followed by hyperplastic polyps(35.7%),inflammatory polyps(9.1%),adenomatous polyp(2.1%)and inflammatory fibroid polyps(1.3%).The most common type of polyps was that diameter of gastric polyps less than 0.5cm,accounting for 70.9%.4.According to Yamada classification,the type Ⅰ gastric polyps were 133 cases(21%),type Ⅱ 472 cases(75.2%),type Ⅲ 21 cases(3.3%),type Ⅳ 2 cases(0.3%).The most common sites of gastric polyps were gastric fundus and body,accounting for 28.8%and 36.6%respectively.ConclusionsFemale were more prone to gastric polyps.Elderly people were more likely to suffer from gastric polyps.The sites of gastric polyps were gastric body,gastric fundus,antrum,cardia,stomach angle,pylorus respectively.The most common size was less than 0.5cm in diameter and>2.0cm was rare.According to Yamada classification,type Ⅱ was the most common in this study.The pathological type of gastric polyps were from more to less:fund gland polyps,hyperplastic polyps,inflammatory polyps,adenomatous polyps and inflammatory fibroid polyps.Adenomatous polyps were more common in male patient,whereas gastric fundus polyps were more common in female.Fund gland polyps was in the gastric fundus and body mostly.Hyperplastic polyps happened more frequently in gastric body and antrum,inflammatory polyps and chronic inflammation were in cardia and antrum.Inflammatory fibroid polyps occurred mainly in the gastric antrum.
