| Objective: To observe the incidence and progression of acute kidney injury of Hepatitis B cirrhosis and its association with clinical parameters.To explore its clinical significance.Methods: Retrospective analysis 148 cases of hepatitis b cirrhosis patients,dividedinto pure liver cirrhosis group,liver cirrhosis with AKI(further stages of AKI aregiven),collected clinical parameters,study the relationship between the incidence andprogression of hepatitis b cirrhosis with AKI with clinical parameters by linearegression analysis.Statistical analysis: Statistical software SPSS21.0 was applied to analyse all data,measurement data using mean±standard deviation(?x±s).Independent sample t test was used to compare the measurement data between the two groups.One way Analysis of Variance and nonparametric test(Kruskal Wallis H test)was used in multiple group(number>2).Multivariate linear regression was used to analyze the incidence and severity of acute renal impairment and its correlation with clinical parameters(P <0.05 was considered statistically significant).Results:1.The incidence of hepatitis b cirrhosis and AKI: 148 cases of hepatitis b cirrhosis were accepted in this study,including 27 cases(18.24%)of hepatitis b cirrhosis with AKI,of which the incidence of AKIⅠstage was 13.51%,AKIⅡstage was 4.73%.2.The correlation analysis between hepatitis b cirrhosis with AKI and clinical parameters: Independent sample t-test was used to compare clinical parameters between the hepatitis b cirrhosis and the hepatitis b cirrhosis with AKI.The result shows: NH3,CK,CKMB,HBDH,CHE,ALB,β2MG,TBIL,BUN,e GFR,PT,INR,APTT,Hgb,PLT,ICG,k Pa,Sex,ascites,Child-Pugh score have the significant statistical difference between the two groups(P < 0.05).The binary linear regression analysis was further used to see the the clinical data and the occurrence of AKI,HBDH,Hgb,e GFR showed significant statistical difference.3.The relationship between clinical parameters and hepatitis b cirrhosis with AKI : Analyzed the clinical parameters to see the relationship in the control group,hepatitis b cirrhosis with AKIⅠand hepatitis b cirrhosis with AKI Ⅱby One way Analysis of Variance test: the NH3,CKMB,HBDH,CHE,ALB,β2MG,TBIL,BUN,e GFR,PT,INR,APTT,Hgb,PLT,ICG,Sex,ascites,Child-Pugh score have the significant statistical difference between the two groups(P < 0.05),further analysis by multiple linear regression,the results showed that e GFR,INR,Age was related with the occurrence and progress of hepatitis b cirrhosis with AKI.Conclusion: With e GFR decreased,INR increased,the incidence and staging increased in hepatitis b cirrhosis with AKI,suggesting that changes in e GFR and INR may be a clinical indicator for the occurrence and progression prediction of hepatitis b cirrhosis with AKI. |
