Objective:Retrospective analysis of clinical characteristics and risk factors of 169 patients with hepatitis B cirrhosis and renal injury.MATERIALS AND METHODS:We collected the data of 169 patients with hepatitis B cirrhosis who were admitted to the Department of Hepatobiliary and Pancreatic Medicine in our hospital from January 2018 to December 2019.Then according to eGFR,we divide the 169 patients into renal injury group and non-renal injury group.Comparing the general characteristics(gender,age,past history,course of cirrhosis),biochemical characteristics(WBC,NE,Hb,PLT,PT,APTT,TT,INR,FBG,ALT,AST,GGT,ALP,TBIL,TBA,albumin,HBV DNA quantification,Child-Pugh classification,urea nitrogen,creatinine,cystatin C,uric acid and so on),Complications(upper gastrointestinal bleeding,hyponatremia,spontaneous bacterial peritonitis,ascites,hepatic encephalopathy,hyperbilirubinemia,hypoalbuminemia)between the two grougs,then screening out different indicators.Using SPSS 22.0 software and multivariate Logistic regression analysis to summarize the independent risk factors for hepatitis B cirrhosis with renal injury.Result:(1)Compared with non-renal injury group,the kidney injury group had higher age levels(P<0.05).(2)Compared with the non-renal injury group,the APTT,ALP,GLB,TBIL,uric acid,creatinine,urea nitrogen,and cystatin C levels were higher in the renal injury group(P<0.05),PLT level was lower in the renal injury group(P<0.05).The incidence of renal injury with eGFR as the limit was higher than that with sCr as the limit(P<0.05),the sensitivity of cystatin C to diagnose renal injury was 93.1%,and the specificity was 91.2%,which was higher than creatinine and urea nitrogen.(3)There were no significant differences in upper gastrointestinal bleeding,hyponatremia,spontaneous bacterial peritonitis,ascites,hepatic encephalopathy,hyperbilirubinemia,and hypoalbuminemia between the two groups(P> 0.05).(4)Age is an independent risk factor for hepatitis B cirrhosis with renal injury(OR = 1.039,P = 0.027),patients with hepatitis B cirrhosis over 53.5 years old have a significantly increased risk of kidney damage,at this time the sensitivity is 41.6% and the specificity is 77.9%.Conclusion:(1)In patients with hepatitis B liver cirrhosis,compared with sCr and BUN,CysC has higher sensitivity and specificity in the diagnosis of kidney damage.(2)Age is an independent risk factor for hepatitis B cirrhosis with kidney injury,patients with hepatitis B cirrhosis over 53.5 years old have a significantly increased risk of kidney damage.
