Study On The Interrelationships And Change Of The Sleep And Quality Of Life Before And After Chemotherapy Among Advanced Non-small Cell Lung Cancer Patients

Objectives:1.To clarify the change of the sleep condition,quality of life,and melatonin level before and after chemotherapy in patients with advanced non-small cell lung cancer.2.Compare the difference and interrelationship of the sleep condition and quality of life in patients with advanced non-small cell lung cancer before and after chemotherapy.3.To explore the relationship between the sleep quality change and the change in melationin level in patients.Methods:Screened for eligible non-small cell lung cancer patients undergoing chemotherapy from December 2016 to December 2017 in The Third Affiliated Hospital of Kunming Medical University,adopted the method of comparison before and after the patient's own,and used the unified questionnaire,survey of the patient's sleep condition and quality of life.Evaluated their sleep condition,quality of life,adverse reaction of chemotherapy and the melatonin level in patients with advanced non-small cell lung cancer before and after chemotherapy,evaluated and analyzed the changes in sleep condition and quality of life dynamicly before and after chemotherapy.The statistical inference was based on one-way ANOVA,correlation analysis,and P<0.05 was considered statistically significant.Results:1.A total of 62 patients was enrolled in this study,at the time of admission,37 ofthe 62 patients(59.7%)had a sleep disorder or a risk of sleep disturbance.At theend of the first chemotherapy,there were 43 cases of sleep disorder(69.4%),and48 cases(79%)at the end of second chemotherapy.2.Compare three times before and after chemotherapy in patients with total scores of PSQI index,was considered statistically significant(P<0.05).Compare the indexs of sleep efficiency score and the sleep disorders score before chemotherapy with at the end of the first chemotherapy and at the end of second chemotherapy respectively,was considered statistically significant(P<0.05).Compare the indexs of sleep time score before chemotherapy with at the end of second chemotherapy,was considered statistically significant(P<0.05).However,the changes of sleep quality,sleep duration,hypnotic drugs,daytime function were not statistically significant after chemotherapy than those before chemotherapy.3.The results of ?2 test showed that nocturnal urination,nocturnal wakefulness or early awakening,and chemotherapy gastrointestinal reaction were the influence factors of sleep in lung cancer patients during chemotherapy.4.Correlation between quality of life and melatonin level in patients and sleep condition:both the five functional status field score and overall level of health score the quality of life questionnaire(EORTC QLQ C30)are negatively correlated with PSQI score,eight symptoms domain score and PSQI scores were positively correlated,and there was no significant correlation between the score of economic difficulties and PSQI score.The content of melatonin in vivo was negatively correlated with the PSQI score.Conclusions:1.The prevalence of sleep disorders was high in patients with non-small cell lung cancer,and with the increase of times of chemotherapy,the patients' sleep disturbance increased significantly.2.Maybe the effects of nocturnal urination,nocturnal wakefulness or early awakening,and gastrointestinal reactions in chemotherapy were the factors affecting sleep during chemotherapy for lung cancer patients.3.Both the quality of life and the melatonin level in vivo had a significant relationship with the sleep condition,and they may have an impact on each other.
