Lung cancer is one of the common malignant tumors in the clinic,which has got the first place of incidence and death rate in most parts of worldwide developed areas.Non-small-cell lung cancer(NSCLC) is the major part of lung cancer,more than 2/3 patients of which has stepped in the advanced stage when finally diagnosed.Nowadays,the combined therapeutic method becomes the main treatment mode of NSCLC patients,which is based on the chemotherapy and radiotherapy added with traditional Chinese medicine.In the past,the evaluation on combined therapeutic effect is standardized with objective curative effect such as variation of the tumor.In the recent decades,following with alternation of the idea of treatment,improvement or elevation of quality of life(QOL) in the clinic evokes widespread concern in oncology domains gradually.Traditional Chinese medicine has the definite effect of improving QOL of patients with NSCLC,so there is a great significance in the clinic to search for effectual medicines on NSCLC.Objectives:To observe short-term effect and QOL about Chinese traditional patent medicine Feitai Capsules(FTC) combined with Chemotherapy on advanced stage NSCLC patients.Methods:A sum total of 60 NSCLC patients inⅢb~Ⅳstage without operations were randomly assigned into 2 groups,30 patients in the combined treatment group given FTC and chemotherapy and 30 patients in the control group treated chemotherapy alone with normal schemes containing cisplatin such as TP/NP.During the two treatment courses,we observe variation of the targeted tumor according to RECIST(Response Evaluation Criteria In Solid Tumors),which has four levels incuding CR(Complete Response),PR(Partial Response),SD (Stable Disease),PD(Progressive Disease).Also we investigate the influence on QOL of NSCLC patients with FTC estimated by EORTC QLQ_C43 and FACT_L questionnaire,the two international questionnaires and traditional Chinese medicine QOL questionnaire amended by us in accordance with the available forms as the tools for measurement,referring to the traditional evaluating indexes of clinical efficacy,such as KPS(Karnovfsky Performance Scoring),ECOG(Eastern Cooperative Oncology Group).The toxicity effect of FTC on patients were observed simultaneously,referring to NCI(national cancer institute) toxicity effect classification criteria.The indexes of our traditional Chinese medicine QOL questionnaire consists of five parts,which contain symptom variation,family life,social work,relationship of doctors and illness,affection function. Results:1.Recent efficacy evaluation:There was no CR patient in the combined treatment group also there was no CR patient in the control group.PR rate was 16.7%in the combined treatment group compared with 13.3%in the control group.SD rate was 43.3%in the combined treatment group compared with 40.0%in the control group.PD rate was 40.0%in the combined treatment group compared with 46.7%in the control group,so no significant discrepancy was shown in the overall response rate between two groups(P>0.05),but significant in the stabilization rate[CR+PR+SD/n(total cases)]about 60.0%in the combined treatment group compared with 53.3%in the control group.2.QOL investigation:Though three questionnaires evaluation QOL in the combined treatment group was improved after treatment with the improvement of scores in majority domains,including symptom sub-domain and functional items,while it in the control group deprived,showing significant difference between the two groups(P<0.05).Similar results were also shown in the evaluation of physical performance by KPS.3.There was good correlation and concordance about symptom variation,family and social work,affection function items between traditional Chinese medicine QOL questionnaire and the two international questionnaires,so in general the self-mended questionnaire possesses good response and reliability coefficient.4.Toxicity effect:The incidence rate of nausea,emesis,constipation and stomatitis such adverse evidences in the control group were significantly higher than those in the treatment group (P<0.05),and the incidence time of all items in the treatment group was less than those in the control group.Conclusions:1.FTC combined with chemotherapy in treating advanced stage NSCLC had some significance of tumor stabilization compared with chemotherapy alone.2.FTC combined with chemotherapy can improve QOL of NSCLC patients more predominancely than chemotherapy alone,shown in symptom variation and functional items which are closely related to QOL.3.The self-mended questionnaire has good concordance with two international questionnaires,which generally possesses good response and reliability coefficient and probably offers some reference to efficacy evaluation of Chinese patent medicine on treating lung cancer.4.FTC combined with chemotherapy reduced the adverse reaction of chemotherapy and improved QOL of the patients indirectly.5.Chinese patent medicine represented with FTC combined with chemotherapy will be a good choice to advanced stage NSCLC patients.
