Efficacy, The Evaluation Of Quality Of Life And Analysis Of Prognostic Factors On Non-small Cell Lung Cancer Accepted Different Treatment Styles

Objective: To explore the efficacy of different treatment styles in order to evaluate the clinical effect and toxicity and to investigate the influence of quality of life of concurrent chemo-radiotherapy and sequential chemo-radiotherapy and chemotherapy for non-small cell lung cancer (NSCLC) . To research the influence of prognostic factors of NSCLC.Methods: 151 patients with NSCLC were divided into three groups: concurrent chemo-radiotherapy group (43 patients) and sequential chemo-radiotherapy group (49 patients) and chemotherapy only group (59 patients) . To evaluate the efficacy of different treatment styles,and patients were analysis by European Organization for Research and Treatment of Cancer's quality of Life Questionnaire-Core (EORTC QLQ-C30) before therapy,after therapy (4th week) and after therapy 1 month. To analysis prognostic factors of NSCLC.Result: (1) The overall response rate in the three group was 81.4%,73.5% and 39%,respectively. The rate of the overall response in concurrent chemo-radiotherapy group and sequential chemo-radiotherapy group were higher than chemotherapy only group (P<0.05); (2) the median survival time was 26 months, 12 months, 11 months,respectively,the median survival time in concurrent chemo- radiotherapy group was longer than the others (P<0.05) ; 1-year survival rate was 60.8%, 47.1% and45.6%, respectively; 2-year survival rate was 58.2%,38.5% and 8.1%, respectively; 3-year survival rate was 32.7%,22.9%,0,respectively.The overall survival rate of concurrent chemo- radiotherapy group was higher than chemotherapy only group (P<0.05),there were no difference in concurrent chemo-radiotherapy group and sequential chemo- radiotherapy group; (3) Hematologic toxicities of concurrent chemo-radiotherapy group and chemotherapy only group was more notable than sequential chemo-radiotherapy group. The rate of radiation esophagitis and radiation pneumonitis was not different in the concurrent chemo-radiotherapy group and sequential chemo-radiotherapy group; (4) There were all downtrends on scorecard in some fields such as body function,role function,emotion function,social function,fatigue, vomiting, nausea,anorexia,but 1 - month later,they are all improved greatly and in chest pain field and total health situation, pain, dyspnea,insomnia,the scorecards also improved greatly in the concurrent chemora- diotherapy group 1-month later. The quality of life of the other groups was better than the concurrent chemo-radiotherapy group 1 day after therapy,but it was worse 1month after therapy; (5) Age,PS and clinical staging were influencing factors in the prognosis of NSCLC.Conclusion: Concurrent chemo-radiotherapy for patients with NSCLC could improve the quality of life,median survival time and the survival rate.It was a safe and effective treatment style,and could be well tolerated. Age, PS and clinical staging system as independent indicators play an important role on the prognosis of patients with NSCLC.
