The Study Of Functional Constipation Associated With Spinal Disorders

ObjectiveBy collecting the relevant information of patients with functional constipation treated by anorectal clinic and inpatient department of Guangdong Provincial Hospital of Traditional Chinese Medicine by the means of questionnaire survey,in order to analyze and summarize the characteristics of the population of functional constipation patients treated in our department,and discuss the proportion of functional constipation with spine-related lesions,the characteristics of the disease and the correlation between them were analyzed.At the same time,the TCM tongue and pulse of the patients with functional constipation were summarized,hope to provide some references for the treatment of constipation.MethodsThe patients diagnosed with functional constipation in the Anorectal Clinic and Inpatient Department of Guangdong Provincial Hospital of Traditional Chinese Medicine from April 2017 to March 2018,were selected as the survey subjects.Through the way of questionnaire and detailed examination to complete the patient’s medical history,symptoms,signs,related to the auxiliary examination and other information collection.The SPSS20.0 statistical software was used to establish a database,than the crowd characteristics,incidence characteristics of functional constipation was analyzed;and the incidence ratio,disease characteristics and relevance of functional constipation patients with spine-related lesions was discussed.At the same time,the tongue picture and pulse condition of functional constipation patients treated by TCM were summarized.ResultsFrom April 2017 to March 2018,348 cases were included,111 men,31.9% of the total,237 women,68.1% of the total,2.14 times the incidence of constipation in women.The body mass index of the patients was fluctuated between 16.6 and 28.6,and 72.41% were in the normal body mass index(BMI).The age of the included constipation patients showed a normal distribution,mainly in the middle-aged people,and the patients who were more than 45-59 years old and more than.60% were not able to use drug or manipulative assistance.The probability of women failing to defecate naturally is higher than that of men;the distribution of body mass index in natural defecation is statistically different,and the probability that thin patients can’t defecate naturally is higher.There are differences in defecation in different age groups.Older people are more likely to be unable to defecate naturally.There was no statistical difference in the distribution of gender in the severity of symptoms.There were 42 patients with spinal related diseases,including 18 males and 24 females.The ratio of males to females was about 1: 1.33.There was no significant difference in gender distribution between patients with and without constipation.About 71.43% of patients with spinal associated diseases were unable to defecate naturally,which was more likely than that of patients without spinal associated diseases.Whether or not combined with spinal related lesions could be discharged naturally.There was no statistical difference in the distribution of defecation frequency.The severity of constipation in patients with spinal associated diseases is higher than that in patients without spinal associated diseases.Of the 42 patients with spinal associated diseases,23 were diagnosed as having spina associated diseases prior to constipation and 10 had been diagnosed with constipation before the onset of constipation There were 9 patients with unwell symptoms of spinal related diseases who could not accurately describe the sequence of constipation and the occurrence of spinal.Of the 42 patients,9 were diagnosed as lumbar disc herniation and 18 as spinal degenerative lesions.But the symptoms or signs of the corresponding spinal segment lesions were found in 6 cases with upper limb numbness,5 cases with upper limb pain and 3 cases with limited cervical movement,1 case with cervical skin pigmentation,11 cases with cervical tenderness,2 cases with chest and back pain,2 cases with numbness,1 case with thoracic back pain,1 case with cervical skin pigmentation and 1 case with thoracic back pain.Pigmentation in 1 case,limited activity of thoracic vertebrae in 4 cases,tenderness of thoracic vertebrae;There were 9 cases of lower limb numbness,9 cases of lower limb pain,9 persons showed lower limb pain and cough could make limb pain more serious,9 cases with lumbar tenderness,7 cases with skin pigmentation in the back of the waist,and 9 cases of straight legs.Nine cases were positive for elevation test and 9 cases for limited lumbar movement.In the aspect of tongue image,the tongue picture of 348 patients was summarized.Among them,22 cases(6.32%)were yellow,36 cases(10.34%)were less tongued,146 cases(41.95)were light red,of which 70 cases were thin and greasy,and 76 cases(21.8%)were thin and white,and 64 cases were mild and fat.There were 25 cases of light and dark tongue(7.18%),20 cases of pale white tongue(5.75%)and 25 cases of other 25 cases(7.18%).It could be seen that nearly half of the patients showed light tongue,or light fat,or light dark,tongue light deficiency.In terms of pulse appearance,22 cases showed pulse slippage,70 cases of which accounted for 6.32% of the total,70 cases showed fine slip,76 cases accounted for 20.11%,76 cases showed fine pulse appearance,36 cases accounted for 21.84% of the total,36 cases showed fine number,and 64 cases accounted for 10.34% of the total,which showed deep delay.Of the total,25 cases(18.39%)showed deep and astringent,20 cases(7.18%)presented sinkiness,10 cases(5.75%)were astringent and 2.87%,the other 25 cases(7.18%)showed fine veins.ConclusionThe age of patients with functional constipation showed normal distribution,mainly in middle-aged patients,which was prone to occur in women.About 10% of patients with constipation complicated with spinal related diseases.The severity of constipation symptom is higher than that without spinal related diseases.About 20% of the patients have definite lumbar disc herniation.Therefore,in the course of consultation,the doctor should inquire and check the body in detail,and find out as soon as possible,and then transfer to orthopedic department for further treatment.In Chinese medicine,constipation patients More for deficiency syndrome,in the treatment of constipation patients with Chinese medicine should pay attention to tonify deficiency.
