| Objective:To observe the effect of Qizhen Jiangtang granule,composed by the guideline of liver-kidney-tonifying,essence-enriching and eyesight-improving,on early diabetic retinopathy and AGES SOD of serum.To investigate the possible mechanism of Qizhen Jiangtang granule on early diabetic retinopathy and the influence of the oxidative stress state in the early diabetic retinopathy model rats.Methods :1.Clinical research: 64 cases of early diabetic retinopathy were randomly divided into control group(32 cases)and treatment group(32 cases).The two groups were treated with the same routine hypoglycemic regimen.Besides,the control group was treated with calcium dobesilate,while the treatment group was administered Qizhen Jiangtang granule on the basis of the control group.A treatment course last 6 courses(with 4weeks in a course)for each group.To observe the change of TCM syndrome integral,fasting blood glucose(FPG),2h postprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c),serum AGEs,SOD content and visual acuity,subhyaloid hemorrhage.To investigate the clinical intervention effect of Qizhen Jiangtang granule on early diabetic retinopathy.2.Experimental research: Seventy male healthy SD rats of SPF grade were randomly divided into control group(n=10)and experimental group(n=60)after one week of adaptive feeding.The rats in control group were fed with common diet,and rats in the experimental group were fed on a high-fat diet.Four weeks later,rats in experimental group were treated with streptozotocin(STZ)injectioncombined with high-fat diet to establish diabetic rat model.If fed with high fat diet for10 weeks,the model of early diabetic retinopathy could be established successfully.The56 rats were randomly divided into 4 groups: the western medicine group(n=14),the model group(n=14),the high dose group(n=14)and the low dose group(n=14).The control group and the model group were perfused with normal saline,and the western medicine group were perfused with calcium dobesilate.The high and low dose groups of Chinese medicine were perfused with different doses of Qizhen Jiangtang granul.All groups were perfused intragastrically for 12 weeks.Observe the diet and defecation,mental state and body weight of rats before and after treatment.After the rats were killed,the FPG and serum ages sod content were measured by abdominal aorta blood sampling.Results:1.Clinical research: clinical observation showed that after 24 weeks of treatment,the total effective rate of treatment group 81.25%,control group total effective rate was57.81%,the difference was statistically significant(P<0.05);after the treatment of TCM syndrome integral of the two groups were significantly decreased than before treatment,and the treatment group than the control group(P<0.05);the two groups after treatment of FPG,2hPG and HbA1 c were lower than before treatment(P<0.01),compared with the control group,the treatment group decreased significantly(P<0.05).Compared with those before treatment,the vision and fundus of the two groups were all improved after treatment,and the blurred vision was slightly improved(P<0.05),and the treatment group was better than the control group.The difference between the two groups was statistically significant(P<0.05).The SOD content in the serum of the treatment group and the control group increased significantly compared with that before treatment,while the AGEs content decreased(P<0.05),and the treatment group was significantly better than the control group(P<0.01).2.Experiment research: animal experiment resultsshowed that compared with the model group,the FPG and HbA1 c of the high and low dose Chinese medicine group and Western medicine group decreased,the difference was statistically significant(P<0.01),and the FPG and HbA1 c of Chinese medicine high and low dose group decreased more obviously.Compared with the control group,the level of group SOD rats decreased significantly,and increased the level of AGEs(P<0.05);compared with the model group,western medicine group,the level of higher dose group rats SOD level,AGEs level decreased(P<0.05).Conclusion:Qizhen Jiangtang granules can effectively improve the visual acuity of the patients with early diabetic retinopathy and fundus condition,improve liver and kidney yin deficiency syndrome.The mechanism of Qizhen Jiangtang granules treating on early diabetic retinopathy might be its regulating role,which it could takes effect by decreasing blood sugar,adjusting glycosylated hemoglobin levels,elevating SOD levels,reducing AGEs accumulation and improving the oxidative stress state. |
